Abstract

Background

The Korean National Healthcare-associated Infections Surveillance System (KONIS) is a nationwide surveillance network established by the Korean Society for Healthcare-Associated Infection and Prevention in July 2006 to perform healthcare-associated infection surveillance using standardized methods. This report presents the annual data of the intensive care unit (ICU) module of the KONIS system between July 2020 and June 2021.

Methods

We performed prospective surveillance of healthcare-associated infections (HAIs), including urinary tract infections (UTIs), bloodstream infections (BSIs), and pneumonia (PNEU), at 339 ICUs in 257 hospitals using the KONIS database. HAI rates and device-associated infection (DAI) rates were calculated as the numbers of infections per 1,000 patient days

(PD) and device days (DD), respectively. Device utilization was calculated as the ratio (DUR) of device days to patient days.

Results

A total of 4,435 HAIs were found during the study period: 1,645 UTIs (1,589 cases were urinary catheter-associated), 1,994 BSIs (1,753 were central line-associated), and 796 PNEUs (383 were ventilator-associated). The rate of urinary catheter-associated UTIs (CAUTI) was 1.30 cases per 1,000 DD (95% confidence interval [CI], 1.24-1.36) and DUR was 0.74 (95% CI, 0.739-0.741). The rate of central line-associated BSIs was 2.21/1,000 DD (95% CI, 2.11-2.31) and DUR was 0.48 (95% CI, 0.479-0.481). The rate of ventilator-associated PNEUs was 0.79/1,000 DD (95% CD, 071-0.87) and DUR was 0.29 (95% CI, 0.289-0.291).

Conclusion

The overall DAI rate was similar to that of the previous year’s data; however, the rate of VAP showed a trend of decline. Furthermore, all DURs were reduced. Therefore, continuous infection surveillance may reduce infection rates and device use.

INTRODUCTION

2019년 12월 코로나바이러스감염증-19 (코로나19)가 출현하였고[1], 국제보건기구(World Health Organizations; WHO)에서 코로나19로 인한 국제공중보건 비상(’20.1.30) 및, 대유행을 선언(’20.3.12) 한 이후[2,3], 모든 감염관리실의 인적 자원과 물적 자원은 대부분 코로나19 비상 대응에 투입되었다. 그럼에도 불구하고, 의료관련감염감시는 환자안전과 의료 질 관리에 필수적인 요소이며, 의료관련감염 예방의 첫 걸음으로써 중요한 활동으로 자리잡고 있다[4 -8]. 특히 의료관련감염은 병원 내 사망의 주요 원인이자 국민보건을 위협하는 중요한 문제로서 적절한 감염관리를 통해 예방하고 관리하기 위한 국가 차원의 노력이 필요하다[9 -11]. 대한의료관련감염관리학회에서 2006년 7월부터 질병관리청의 지원을 받아 운영하는 전국의료관련감염 감시체계(Korean National Healthcare-associated Infections Surveillance System, KONIS)는 우리나라의 대표적인 의료관련감염 감시체계로써, 대상 병원의 높은 참여율과 표준화된 방법을 통해 신뢰도 높은 중재활동 기초 자료로 자리매김하고 있다[12,13]. 이 보고서는 2020년 7월부터 2021년 6월까지의 KONIS 중환자실 감염 감시의 열네 번째 연간 보고이자, 대유행의 파고 속에서도 묵묵히 감염감시를 수행한 뚝심의 기록이다.

MATERIALS AND METHODS

2020년 7월부터 2021년 6월까지 150병상 이상의 총 257개 병원이 참여하였으며, 각 참여병원의 성인 중환자실 중 대표적인 내과계와 외과계 중환자실 또는 통합 중환자실을 대상으로 하여 339개 중환자실이 감시에 참여하였다. 각 참여병원의 형태적 특성, 인력 구성 등 기초정보를 조사하였다. 2019년부터 참여병원이 150병상 이상 규모의 병원으로 확대되면서 병상 규모에 따라 150-299, 300-499, 500-699, 700-899병상, 900병상 이상의 다섯 군으로 분류하여 분석하였다. 중환자실의 분류는 해당 중환자실 병상의 평균 진료과별 환자 구성에 따라 하였으며, 이전 보고와 동일하다[14 -17].

연구 기간 동안 각 참여 중환자실에서 요로감염, 혈류감염, 폐렴에 대한 의료관련감염 감시를 수행하였고, 중환자실 의료관련감염과 요로감염(urinary tract infection, UTI), 혈류감염(bloodstream infection, BSI), 폐렴(pneumonia)의 정의는 미국 질병관리본부(Center for Disease Control and Prevention, CDC)의 정의에 근거하여[18] 개정한 KONIS 진단기준을 적용하였다. 요로감염은 ‘증상이 있는 요로감염(symptomatic urinary tract infection, SUTI)’과 ‘무증상 균혈증 요로감염(asymptomatic bacteremic urinary tract infection, ABUTI)’을 포함하였고 혈류감염은 ‘검사로 확인된 혈류감염(laboratory-confirmed bloodstream infection)’을 감시대상으로 하였다. 폐렴은 ‘임상적 폐렴(clinically defined pneumonia, PNEU1)’, ‘특이적 검사소견으로 정의한 폐렴(pneumonia with specific laboratory findings, PNEU2)’과 ‘면역저하자 폐렴(pneumonia in immunocompromised patients, PNEU3)’으로 분류하였다. 요로감염, 혈류감염과 폐렴에서 각각 유치도뇨관, 중심정맥관, 인공호흡기와의 관련 여부를 구분하였다. 의료관련감염률은 재원일수(patient-days, PD) 1,000일당 의료관련감염 건수, 기구일수감염률은 기구일수(device-days, DD) 1,000일당 기구관련감염 건수, 기구사용비는 기구일수/재원일수로 계산하였다. 항생제 감수성 결과가 있는 주요 세균을 대상으로 항생제에 대한 내성률을 조사하였고 각 항생제의 감수성 결과에서 S는 감수성(susceptible), I는 중등도 내성(intermediate), R은 내성(resistant)을 의미한다. 중등도 내성은 내성에 포함하여 분석하였고, 따라서 보고된 의료관련감염을 일으킨 원인 미생물 중, 해당 균주의 항생제 감수성 결과가 있는 총 건수와 I와 R로 보고된 해당 균주의 건수의 백분율로 내성률을 조사하였다. KONIS 의료관련감염감시 기간동안 기구일수감염률의 변화를 비교 및 분석하기 위해 2020년 7월-2021년 6월 기간의 기구일수감염률 자료를 2016년 7월-2020년 6월 사이의 최근 4년간의 연간자료와 비교하였다[15 -17,19]. 기구일수감염률 및 기구사용비의 95% 신뢰구간(confidence interval, CI)이 겹치지 않는 경우에 유의한 차이가 있는 것으로 판단하였다.

RESULTS

1. KONIS 참여병원과 참여중환자실의 특성

2020년 7월부터 2021년 6월까지 KONIS 중환자실 부문에 참여한 병원의 참여 기준은 150병상 이상 병원으로서 중환자실을 운영하는 의료기관이며, 257개 기관 339개 중환자실에서 보고하였으며, 중환자실의 특성은 Table 1과 같다. 전국 대상 기관 319개 중 80.6%의 참여율을 보였으며, 전체 참여병원의 64.2% (165개)가 500병상 이하 규모의 병원이었다. 총 참여 병원 중 전공의 및 의대학생 실습 수련병원인 주교육병원은 77개(30.0%)였고, 평균 병상 수는 504개였다. 내과계열 중환자실의 비율이 72.2%로 여전히 높았으며, 각 구성은 Table 1에 보고된 바와 같다[17].

2. 중환자실 의료관련감염률

연구 기간 동안 1,657,370 patient day가 감시되었으며, 총 4,435건의 의료관련감염이 보고되었다. 감염 종류별로는 혈류감염이 1,994건(45.0%)으로 가장 많았고 요로감염 1,645건(37.1%), 폐렴 796건(17.9%) 순이었다. 전체 의료관련감염률은 2.68/1,000 PD (95% CI, 2.60-2.76)로 전년도2.83/1,000 PD (95% CI, 2.75-2.91)에 비해 낮게 보고되었다. 요로감염 발생률은 0.99/1,000 PD (95% CI, 0.95-1.04), 혈류감염 발생률은 1.24/1,000 PD (95% CI, 1.15-1.26)로 전년도(1.04/1,000 PD [95% CI, 0.99-1.09], 1.24/1,000 PD [95% CI, 1.18-1.29])와 큰 차이를 보이지 않았으나, 폐렴 발생률은 0.48/1,000 PD (95% CI, 0.45-0.51)로 전년도 자료인 0.55/1,000 PD (95% CI, 0.52-0.59)와 비교할 때 유의미한 감소를 보였다. 병상의 규모별로 비교했을 때, 700-899병상에서는 의료관련 요로감염과 혈류감염 발생률이 증가 경향을 보이면서 전체 의료관련 감염률도 유의하게 증가하였으나, 500-699 병상에서는 혈류감염과 폐렴의 두드러진 감소로 전체 의료관련 감염률이 유의하게 감소하였다(Table 2) [17].

3. 기구관련 의료관련감염률과 기구사용비

유치도뇨관 관련 요로감염(urinary catheter-associated UTI, CAUTI)은 의료관련 요로감염 1,645건 중 1,589건(97.0%)이었다. 전체 유치도뇨관 기구일수는 1,222,877일이었고, 유치도뇨관 관련 요로감염 발생률은 1.30/1,000 DD (95% CI, 1.24-1.36)로 전년도(1.26/ 1,000 DD [95% CI, 1.20-1.32])와 비교해서 유의하지는 않으나 증가경향을 보였다(Table 3). 유치도뇨관 사용비는 0.74 (95% CI, 0.739-0.741)로 전년도(0.80 [95% CI, 0.799-0.801]) 대비 감소하였다(Table 4). 증심정맥관 관련 혈류감염(central line-associated BSI)은 의료관련 혈류감염 1,994건 중 1,753건(87.9%)이었으며, 발생률은 2.21/1,000 DD (95% CI, 2.11-2.32)로 2019년(2.16/ 1,000 DD [95% CI, 2.06-2.26])와 비슷하게 2018년(2.32/1,000 DD [95% CI, 2.22-2.43])에 비해 감소 경향을 보였다. 전체 중심정맥관 기구일수는 793,009일이었으며, 중심정맥관 사용비는 0.48 (95% CI, 0.479-0.481)로 2019년(0.50 [95% CI, 0.499-0.501])에 비하여 증가하였다. 인공호흡기관련 폐렴(ventilator-associated PNEU)은 전체 의료관련 폐렴 796건 중 383건(48.1%)을 차지하였고, 인공호흡기 관련 폐렴 발생률은 0.79/1,000 DD (95% CI, 0.71-0.87)로 2019년(0.93/1,000 DD [95% CI, 0.85-1.02])과 비교하여 감소 경향을 보였다. 전체 인공호흡기 기구일수는 487,650일이었고, 인공호흡기의 기구사용비는 0.29 (95% CI, 0.289-0.291)로 전년도(0.32 [95% CI, 0.319-0.321]) 대비 감소하였다(Table 3, 4) [17].

4. 병상규모에 따른 기구관련 의료관련감염률과 기구사용비

유치도뇨관 관련 요로감염률은 전년도와 유의미한 차이를 보이는 병상 규모는 없었으며, 유치도뇨관 사용비는 500-699병상(0.57 [95% CI, 0.569-0.571])과 150-299병상(0.64 [95% CI, 0.638-0.642])에서 2019년(0.70 [95% CI, 0.698-0.702], 0.74 [95% CI, 0.738-0.742])에 비해 두드러진 감소율을 보였다. 중심정맥관 관련 혈류감염률은 여전히 500병상 이상의 병원에서 150-499병상 규모의 병원에 비해 높은 감염률을 보였으며, 전년도와 유의미한 차이를 보이는 병상 규모는 없었다. 중심정맥관 기구사용비는 500-699병상(0.39 [95% CI, 0.389-0.391])과 150-299병상(0.30 [95% CI, 0.298-0.302])에서는 전년도 대비(0.46 [95% CI, 0.458-0.462], 0.32 [95% CI, 0.318-0.322]) 유의미한 감소를 보였고, 이외의 규모에서는 모두 약간 증가하였다. 인공호흡기 관련 폐렴 발생률 역시 병상규모별로 전년도와 유의미한 차이는 없었으나, 인공호흡기 사용비는 900병상 이상 규모(0.52 [95% CI, 0.518-0.522])에서 2019년(0.50 [95% CI, 0.498-0.502])에 비해 증가하였으며, 500-699병상(0.24 [95% CI, 0.239-0.241])과 150-299병상(0.13 [95% CI, 0.129-0.131])에서는 전년도 대비(0.31 [95% CI, 0.308-0.312], 0.14 [95% CI, 0.139-0.141]) 유의하게 감소하였다(Table 3, 4) [17].

5. 중환자실 유형에 따른 기구관련 의료관련감염률과 기구사용비

유치도뇨관 관련 요로감염 발생률은 각 중환자실 유형별 유의미한 변화는 없었으며, 여전히 외과계 중환자실이 내과계 중환자실에 비해 높은 경향을 보였다. 중심정맥관 관련 혈류감염 발생률 역시 이전과 같이 내과중환자실(2.75/1,000 DD [95% CI, 2.56-2.96])에서 다른 유형의 중환자실에 비해 유의하게 높았으며, 전년도와의 감염률 간의 유의미한 변화는 없었으나 외과계 통합중환자실(2.31/1,000 DD [95% CI, 2.03-2.61])에서 2019년(2.02/1,000 DD [95% CI, 1.77-2.30])에 비해 약간의 증가 경향을 보였다. 인공호흡기 관련 폐렴 발생률은 외과계 중환자실 유형들에서 내과계 중환자실 유형들에 비해 높은 경향이 유지되었으나 전년도 대비 유의미한 변화는 보이지 않았다. 내과계 통합중환자실(0.68/1,000 DD [95% CI, 0.57-0.82])에서 인공호흡기관련 폐렴 발생률이 전년도(0.94/1,000 DD [95% CI, 0.80-1.10]) 대비 감소경향이 지속되었으며, 신경외과 중환자실을 제외하고 대부분 감소경향을 보였다(Table 5) [17].

유치도뇨관 사용비는 내과 중환자실(0.70 [95% CI, 0.699-0.701]), 내과계 통합중환자실(0.75 [95% CI, 0.749-0.751]) 및 외과계 통합중환자실(0.63 [95% CI, 0.628-0.632])은 지속적으로 매년 유의미한 사용비 감소를 보였다. 전년도와 유사하게 신경외과 중환자실에서 가장 높은 유치도뇨관 사용비(0.90 [95% CI, 0.898-0.902])를 보였으며, 중심정맥관 사용비는 외과 중환자실(0.65 [95% CI, 0.647-0.653])에서 가장 높았다. 또한 외과 중환자실과 신경외과 중환자실(0.53/1,000 DD [95% CI, 0.527-0.533])에서는 중심정맥관 사용비가 지속적으로 전년도(0.62/1,000 DD [95% CI, 0.617-0.623], 0.51/1,000 DD [95% CI, 0.507-0.513]) 대비 유의한 증가를 보였다. 인공호흡기 사용비는 외과 중환자실(0.40 [95% CI, 0.397- 0.403])과 신경외과 중환자실(0.33 [95% CI, 0.327-0.333])에서 전년도(0.39/1,000 DD [95% CI, 0.387-0.393], 0.32/1,000 DD [95% CI, 0.317-0.323]) 대비 유의한 증가를 보였으나, 다른 유형의 중환자실의 인공호흡기 사용비는 전년도에 비해 모두 뚜렷하게 감소하였다(Table 6) [17].

6. 원인미생물과 주요 미생물의 항생제 내성률

중환자실 의료관련감염의 원인 미생물은 총 4,500균주가 분리되었으며, 그람음성막대균 2,180 (48.4%), 그람양성알균 1,797 (39.9%), 진균 385 (8.6%)의 순이었다. 분리된 미생물을 Table 7에 기술하였다. 요로감염의 원인균은 그람음성막대균이 56.6%, 그람양성알균이 40%였으며 Enterococcus faecium (21.5%), Escherichia coli (20.8%), Klebsiella pneumoniae (12.2%), Enterococcus faecalis (10.9%), Pseudomonas aeruginosa (9.7%) 순의 빈도를 보였다. 혈류감염은 그람양성알균이 45.1%로 가장 흔하게 분리되었고 그람음성막대균 34.6%, 진균 17% 순이었다. E. faecium (17.4%), Candida spp. (16.6%), coagulase negative staphylococci (12.7%), Acinetobacter baumannii (11.3%), Staphylococcus aureus (7.8%) 순으로 보고되었다. 진균 중 가장 흔한 원인균은 Candida albicans (41.5%, 158/381)였다. 폐렴에서는 그람음성막대균이 82.2%, 그람양성알균이 16%였으며, 흔한 원인균은 A. baumannii (26.7%), P. aeruginosa (17.4%), K. pneumoniae (16.6%), S. aureus (15.0%), Stenotrophomonas maltophilia (6.1%) 순서로 확인되었다(Table 7) [17].

주요 세균의 주요 항생제에 대한 내성률을 Table 8에 기술하였다. Methicillin 내성 S. aureus (MRSA)는 69.6%로 전년도(75.4%)에 비해 감소하였다. Vancomycin 내성 E. faecium의 비율은 58.8%로 전년도 56.6%에 비해 지속적인 증가를 보였다. A. baumannii의 imipenem 내성률은 92.5%로 전년도 89.7%에 비해 증가하였으며, K. pneumoniae의 imipenem 내성률 역시 27.8%로 이전년도(2019;23.5%, 2018;18.3%)와 비교하여 지속적으로 증가하였다. 광범위 베타락탐항생제 내성을 시사하는 cefotaxime 내성률은 E. coli에서 49.2%로 전년도(52.6%) 대비 감소하였으며, K. pneumoniae에서 66.2%로 전년도(65.2%)보다 높게 보고되었다. E. coli와 K. pneumoniae의 ciprofloxacin 내성률은 64.7%와 64.1%로 전년도(60.8%, 61.0)와 비교해 증가하였다(Table 8) [16,17].

7. 2016-2021년도의 기구사용비와 기구관련 의료관련감염률

2020년 7월부터 2021년 6월까지 1년간의 기구사용비와 기구일수 의료관련감염률을 이전 4년간의 KONIS 자료와 비교하였다. 2016년부터 지속적으로 증가하는 경향을 보였던 유치도뇨관 관련 요로감염 발생률이 전년도와 비교해서는 유의미하지는 않았으나 2016-2018년과 비교해서는 유의한 증가가 확인되었다. 중심정맥관 관련 혈류감염률은 이전 4년간의 자료와 비교할 때 유의한 차이가 없었다. 인공호흡기 관련 폐렴 발생률은 이전 4년간에 비해 두드러지게 유의미한 감소를 보였다. 전년도와 유사하게 올해 보고에서는 모든 기구 사용비는 감소하였으며, 특히 유치도뇨관 사용비와 인공호흡기 사용비가 유의하게 감소하였다(Table 9) [15 -17,19].

DISCUSSION

전국의료관련감염감시체계(KONIS)는 전국에서 자발적으로 참여하는 병원들에 의해 2006년부터 꾸준히 운영되어 왔다[10,13]. 특히 KONIS 중환자실 감염감시(intensive care unit surveillance, KONIS-ICU)는 2006년 7월부터 의료관련감염 감시를 시작하여 2007년 7월부터 연간자료를 발표하기 시작하였으며, 이번 결과보고는 KONIS에 참여한 257개 병원의 339개 중환자실 감시를 수행한 2020년 7월부터 2021년 6월까지의 자료를 정리한 열네 번째 연간자료이다. 또한 2016년부터의 참여병원, 특히 중소병원의 급격한 증가와 참여기간 확대로 인한 변화 이후 다섯번째 보고이다[19].

2019년 7월부터 참여기준이 150병상 이상으로 확대된 이후, 전체 참여 가능한 319개 대상 기관 중, 80.6% (257/ 319)가 참여하였으며, 전년도 참여기관 256개 기관에서 257개로 1개 기관이 증가하였고, 900병상 이상, 700-899병상 규모 0개, 500-699병상 규모 병원 1개 및 300-499병상 규모에서 3개 증가, 100-299병상 규모에서 3개가 감소하였다. 전년도의 참여병원 분포와 비슷하게 유지되었으며, 참여중환자실은 전년도 340개에서 339개로 1개가 감소하였고, 참여병원의 평균 병상 수 역시 전년도 502병상에서 504병상으로 큰 변화를 보이지 않았다. 또한 감염관리전담인력 1인당 병상은 2019년 133병상과 동일하였으며, 여전히 병원당 감염내과 전문의 수는 2015년 1.5명에서 2016년부터 감소한 이후 현재까지도 병원당 0.9명으로 변화가 없었다[17,19].

중환자실 의료관련감염은 4,435건으로 전년도 4,489건에 비해 1.2% 감소하였으며, 전체 의료관련감염률 역시 전년도 2.83/1,000 PD (95% CI, 2.75-2.91)에 비해 감소한 것으로 보고되었다. 이 중에서도 폐렴의 유의한 감소가 두드러졌으며, 특히 699병상 이하의 모든 규모의 병원에서 감소 경향을 보였다. 주목할 점은 전년도와 동일하게 500-699병상 규모의 병원에서 전체 의료관련감염률의 감소가 두드러지고 있고, 특히 혈류감염과 폐렴이 감소된 것이다[17]. 폐렴의 감소는 중소병원에서의 코로나19로 인한 중환자실 환자군의 변화와, 마스크 착용으로 인한 호흡기 감염의 감소가 영향을 미쳤을 것으로 추정되며, 국내 및 국외에서 이와 이에 대한 연구가 활발히 이루어지고 있다. 실제 국내 연구에서 코로나19 대유행 기간동안 폐렴으로 인한 입원률 감소가 확인되었으며, 호흡기 바이러스 및 계절인플루엔자 분리율 역시 감소함이 보고된 바 있다[20 -22]. 국외 연구에서는 오히려 혈류감염과 폐렴을 포함한 의료관련감염률의 증가를 보고한 바 있어[23 -26], 국내에서의 감소가 지속적인 감염감시의 효과인지, 국외와 다른 코로나19 정책 및 중환자실 운용체계에 따른 결과인지 추가 분석이 필요해 보인다.

기구관련 의료관련감염률 자료에서 유의미한 변화는 두드러지지 않았으나, 2015년 이후 지속적으로 증가 경향을 보이고 있었던 유치도뇨관 관련 요로감염 발생률은 2018년 이후 뚜렷하게 증가한 것이 확인되었다[16,17]. 유치도뇨관 관련 요로감염 발생률의 세부분석에서 여전히 신경외과 중환자실(1.93/1,000 DD [95% CI, 1.68-2.21])에서 가장 높은 감염률을 보였으며, 700-899병상 규모 역시 높은 감염률을 보였으나, 규모별이나 중환자실 유형별로 유의미한 변화는 확인되지 않았다. 다만 700-899병상 규모의 경우 전년도와 비교해서 유의하지는 않으나 증가경향이 두드러졌고, 2018년과 비교하면 뚜렷한 증가가 확인되었다[16,17]. 전체 유치도뇨관 사용비는 전년도 대비 뚜렷하게 감소하였으며, 세부분석에서는 500-699병상과 150-299병상에서 유의한 사용비 감소가 확인되었다. 또한 내과계 중환자실 및 외과계 통합 중환자실에서도 유의하게 사용비가 감소하였다. 의료기관들의 삽입기구 관리 노력에 따른 전체 사용비의 감소에도 불구하고 지속적인 감염률 증가 경향을 보이고 있어, 요로감염의 발생에 대한 지속적인 관찰과 중재가 필요할 수 있겠다. 또한 500병상 이하에서 두드러진 기구 사용비 감소를 보이고 있어, 이는 지속적인 중재 및 감염감시의 효과일 뿐 만 아니라 코로나로 인한 중환자실 운영의 변화 역시 영향이 있었을 것으로 추정된다. 대유행 기간 이후의 변화에 대해 추가 분석으로 영향을 확인할 수 있겠다.

중심정맥관 관련 혈류감염 역시 2015년 이후 지속적으로 증가 경향을 보였으나, 2019년부터는 증가를 보이지 않았다. 특히 이는 전체 중심정맥관 사용비가 증가했음에도 감염률이 증가를 보이지 않아, 지속적인 감시체계를 통한 효과로 추정할 수 있겠다(Table 9). 중심정맥관 관련 혈류감염의 세부분석에서 병상규모별로는 이전과 큰 차이를 보이지 않았으며, 중환자실 유형에 따른 분석에서 외과계 통합 중환자실에서의 증가경향 이외에 뚜렷한 변화는 없었다. 중심정맥관 사용비는 유치도뇨관 사용비와 같이 500-699병상과 150-299병상에서 유의한 사용비 감소가 확인되었다. 그러나 외과 중환자실 및 신경외과 중환자실에서의 중심정맥관 사용비는 지속적으로 증가하고 있어 이에 대한 관찰 및 중재가 필요하겠다[16,17]. 또한 국외에서의 대부분 연구에서 코로나19 대유행 기간 중 두드러진 의료관련 혈류감염의 증가를 보고함에 비해, 이번 KONIS 보고에서는 증가경향을 보이지 않은 것은 지속적인 감염관리체계의 유지와 감염관리예방 노력의 효과이겠다. 특히 코로나19 대유행으로 인해 중환자실 운용에 대한 부담과 의료진의 Burn-out이 심해졌을 시기임에도 감염관리가 잘 유지되었음을 의미하는 결과라 할 수 있겠다[6,23,27]. KONIS 자료에서 중증 코로나19 환자들을 따로 구분하여 감시하지는 않았으며, 전담치료병원의 경우 감시에서 제외한 경우도 있어 간접적인 파급효과로 해석할 수 있겠으나, 정확한 세부분석이 필요한 부분이겠다.

인공호흡기 관련 폐렴 발생률은 전년도와의 비교에서 유의하지는 않았지만 감소 경향을 보였으며, 2018년에 비해 유의하게 감소하였다. 또한, 인공호흡기 기구사용비 역시 2018년부터 지속적으로 감소하였다(Table 9) [16,17]. 세부분석에서 인공호흡기 관련 폐렴 발생률은 699병상 이하 규모의 중소 병원에서 주로 감소경향을 보였으며, 내과계 통합 중환자실에서도 지속적인 감소경향을 보고하였다. 인공호흡기 기구사용비 역시 500-699병상과 150-299병상에서 감소하였고, 내과계 중환자실과 내과계 통합 중환자실에서 사용비의 감소가 확인되었다. 따라서 내과계 중환자실들에서의 폐렴에 대한 지속적인 감시와 관리 효과로 인한 감염률과 기구 사용비 감소 효과를 추정할 수 있으나, 여전히 외과계 중환자실에는 변화가 미미한 것으로 보인다. 인공호흡기 관련 폐렴 발생률은 신경외과 중환자실에서 가장 높았으며, 외과 중환자실과 신경외과 중환자실에서의 기구사용비는 증가한 것으로 보고되어, 외과계 중환자실에 대한 중재가 필요할 것으로 보인다(Table 5, 6) [17].

주요 원인 미생물의 분포에서 요로감염의 원인균은 유치도뇨관 장기 집락과 2016년 이후 요로감염 진단기준의 변화로 인해 2018년 이후부터 E. faecium (21.5%)이 가장 흔한 균주로 보고되었으나, 전년도에 비해 E. coli와 K. pneumoniae가 증가한 것으로 보고되었다. 혈류감염에서도 역시 E. faecium (17.4%)이 가장 흔하게 분리되어 두드러진 증가를 보였으며, coagulase negative staphylococci의 분리도 증가한 것으로 보고되어, 중심정맥관 감염관리에 대한 중재가 필요함을 시사할 수 있겠다. 폐렴의 원인균으로는 A. baumannii (26.7%)이 가장 높은 비율로 분리되었으며, P. aeruginosa가 유의한 증가가 특징적이었으며 S.maltophilia도 지속적으로 높은 빈도를 차지하고 있어 내성률이 높고 치료제 선택이 어려운 그람음성 막대균의 분리가 많은 것으로 확인되고 있다. 주요 미생물의 항생제 내성률 결과에서도 MRSA는 69.6%로 지속적으로 감소하고 있으나, A. baumannii의 imipenem 내성률은 92.5%, K. pneumoniae의 imipenem 내성률은 27.8%로 그람음성 막대균의 광범위 항생제 내성률이 급격한 증가를 보이는 것으로 확인되어 적극적인 중재가 필요하겠다[16,17]. Kor-GLASS (Global Antimicrobial Resistance Surveillance System in Korea) 결과와 비교해서도, 병원감염 유래 MRSA는 65.4%, Carbapenem 내성 A. baumannii가 91.2%로 KONIS자료와 비슷하였다. MRSA분리율이 감소 추세이나 여전히 38개국 중 5번째로 높은 것으로 확인되었으며, Carbapenem 내성 A. baumannii의 내성률 역시 35개 국가중 12번째로 높은 것으로 보고되었다[28]. 따라서 다제내성균에 대한 집중적인 관리와, 항생제 관리 프로그램(antibiotic stewardship program, ASP)의 활성화가 시급하겠다[29,30].

2020년 7월에서 2021년 6월까지 중환자실 의료관련감염 자료를 분석한 결과 요로감염은 1,000재원일당 0.99건(95% CI 0.95-1.04), 혈류감염은 1.20건(95% CI 1.15-1.26), 폐렴은 0.48건(95% CI 0.45-0.51)이 발생하였으며, 전년도에 비해 폐렴이 감소하였다. 기구관련 의료관련감염률은 유의미한 변화는 없었고, 모든 기구사용비는 감소하였다. 그리고 그람음성균의 광범위 항생제 내성률 증가가 확인되었다. 대유행 시기임에도 의료관련감염률의 증가 경향은 보이지 않았다. 지속적이고 적극적인 감시 및 중재를 통해 기구관련 의료관련 감염률의 감소 유도와 항생제 내성균 관리가 필요하겠다.
