학대 아동 대상 중재에 대한 통합적 문헌고찰: 국내·외 학대 아동 중재 관련 연구(2009–2019)를 중심으로

Abstract

Purpose

This study aimed to determine the current state of interventions for abused children and their effectiveness by using a systemic review.

Methods

Published intervention studies were identified through MEDLINE, Embase, CINAHL, DBpia, RISS, and Kiss DB. Eligible studies included articles published between 2009 and 2019 in English and Korean.

Results

A total of 28 studies were selected for this review. The intervention elements of 28 selected studies were categorized into 4 themes. The extracted intervention elements were divided into the themes of patient & situation-specific intervention, strengthen parent-child coping capacity and interaction, applying group intervention and integrated intervention, Linkage with community resource and multidisciplinary approach.

Conclusions

The results demonstrate that social and psychological health of abused children has positively changed through these interventions. Therefore, it is important to implement treatment through active intervention aimed at helping the affected children.

서 론

1. 연구의 필요성

1999년 세계보건기구(World Health Organization)는 아동학대를 “아동의 양육에 책임을 지고 있거나 권위를 가진 사람에 의한 모든 형태의 신체·정서적 가혹한 처사, 성학대, 방임, 상업적이거나 다른 형태의 착취에 의해 그 결과 아동의 건강, 생존, 발달 및 존엄성에 실제적인 해를 끼치거나 그런 가능성을 초래한 행위”로 정의하였다(World Health Organization, 1999). 세계의 모든 아동은 학대로부터 보호받을 권리가 있다. 그러나 아동은 자신을 보호할 수 있는 능력이 없고 반복되는 학대를 수동적으로 받아들이는 경우가 많아 학대사실을 인식하지 못하는 악순환을 겪을 수 있다. 학대를 경험한 아동은 신체적, 심리적, 행동적, 정서적 차원에서 장·단기에 걸쳐 부정적 영향을 받게 된다(Mathews et al., 2008).
미국에서 최초로 사용된 아동학대라는 용어는 그 의미가 신체적 학대에 국한되었으며, 신체적 학대는 의도적으로 신체적 손상을 입히는 모든 행위로 구타, 폭력, 화상, 기능의 손상 등을 의미한다(Ju, 2008). 이후 시대가 흐르며 아동학대의 개념은 신체적 학대에서 점차 확대되어, 부모가 폭력을 사용하지 않더라도 아동을 방임하거나 폭언 등에 의해 상처받는 심리적 학대까지 아동학대로 고려되었다. 이러한 정서적 학대는 명확한 정의를 내리기는 어렵지만, 아동에게 협박, 언어적 폭력, 모욕감이나 수치심을 갖게 하는 양육행동을 포함한다. 정서적 학대의 영향은 외적으로 크게 드러나지 않을 수 있지만 아동의 심리적 적응과 정서적 발달에 지대한 영향을 미친다(Ahn et al., 2003). 방임은 부모나 보호의 책임이 있는 성인이 고의적이고 반복적으로 아동을 적절하게 양육하거나 보호하지 않는 행위이다. 이러한 방임은 보호자의 무관심으로 인해 아동의 건강과 발달들을 위협하는 모든 행위를 의미한다. 1980년대 들어 학대의 범위가 더욱 넓어져 아동을 대상으로 하는 성적 행위 또한 성적 학대로 아동학대 범주에 넣었다. 성적 학대는 성인의 성적 요구나 욕망을 위해 아동을 이용한 경우로, 신체적 접촉이나 상호작용을 모두 일컫는다(Kwon, 2003). 이처럼 아동학대는 ‘신체적 학대–방임–심리적 학대–성적학대’의 순서로 그 개념이 확대되어 왔으며(Moon, 2011), 아동의 복지와 발달을 위협하는 넓은 범위의 행위는 모두 아동학대에 대한 광의의 의미로 정의될 수 있다. 특히 아동학대의 경험은 성인이 된 후에도 장애를 유발할 만큼 정신적으로 큰 타격을 주기 때문에 학대경험으로 인한 심리적·정서적 문제를 다루며 중재하고 치료하는 것은 아동학대 개입의 중요한 부분이라 할 수 있다.
국내에서도 2000년 아동복지법이 개정되면서 아동학대 문제에 관심을 갖고 본격적으로 개입하기 시작했으며, 최근 여러 아동학대 사례가 언론보도를 통해 언급되면서 아동학대에 대한 심각성과 학대 발생 후의 중재에 대한 국민의 관심이 커지고 있다. 그러나 아동학대 피해자에 대한 중재의 중요성에도 불구하고 현재 국내·외 연구 중 아동학대에 대한 영향과 관련하여 종합적으로 고찰한 것은 Holt 등(2008)의 연구와 Kim (2012)의 연구 외에는 찾아보기 어렵다. 따라서 Holt 등(2008)Kim (2012)의 연구 이후 10여년이 흐른 현 시점에서 아동학대 대상자를 대상으로 한 중재 연구에 대한 전반적인 고찰이 필요하다고 볼 수 있다. 통합적 문헌고찰은 Whittemore와 Knafle (2005)이 제시한 문헌고찰 방법으로써 5가지 단계, 즉 문제 인식, 문헌 검색과 선정, 자료의 평가, 자료 분석 및 의미 해석, 자료 통합을 속성 도출의 순서를 따랐다. 이는 최근 주로 사용되는 체계적 문헌고찰 방법과는 다르게 다양한 연구방법을 사용한 문헌들을 통합할 수 있다. 이 방법론은 메타분석과 같이 양적인 수치로 표현될 수 없어 통합된 결과의 객관성 확보에 어려움이 있으나 아동학대에 대한 중재의 특성상 다양한 종류의 아동학대를 경험한 대상자와 여러 환경에서 중재가 이루어지므로 특정 결과변수를 중점으로 결과를 통합하기 보다는 여러 연구에서 분석결과를 종합적으로 제시 할 필요가 있다고 판단하였다.
2%–3%에 불과한 의료인의 아동학대 신고율 중 간호사의 신고율이 특히 매우 낮은 편이다. 이는 아동학대 사례를 발견하고 중재하는 것은 중요한 간호과정임에도 불구하고 그에 대한 지식 수준이나 준비정도, 중재방법에 대한 자신감 수준이 매우 낮기 때문으로 보고되었다(Kim & Park, 2005). 이 연구에서는 아동학대에 대한 전문적이고 효과적인 중재와 치료방법을 모색하기 위한 준비로 2009년 1월 1일부터 2019년 12월 31일까지 진행된 국내·외 아동학대 관련 중재 연구를 통합적으로 고찰하여 파악하였다. 이러한 아동학대 중재과정에 대한 이해는 국내 실정을 고려한 아동·청소년 학대 관련 간호중재 프로그램을 개발뿐 아니라 전문적 간호중재 역할에 대한 기초 자료가 되어 아동학대에 대한 간호사의 자신감 향상과 관심을 도모할 수 있을 것이다.

2. 연구 목적

이 연구의 목적은 2009년 1월 1일 이후 아동학대 관련 국내·외 중재연구를 분석하고, 그 특성을 비교하고 확인하는 것이다. 구체적인 목적은 아래와 같다.

연구대상 및 방법

1. 연구 설계

이 연구는 통합적 문헌고찰 연구로서 아동학대 관련 중재 연구에서 사용된 중재를 고찰하고 중재의 속성을 확인하였다. 통합적 문헌고찰의 단계는 Whittemore와 Knafle(2005)이 제시한 (1) 문제 인식 (2) 문헌 검색과 선정 (3)자료의 평가 (4) 자료 분석과 의미 해석 (5) 자료 통합을 통한 속성 도출의 순서로 진행하였다. 자료 통합을 통한 속성 도출 단계는 자료를 추출하고 통합적 측면에서 비교·분석하는 것으로 추상화된 새로운 개념을 도출하는 것이다. 이 연구에서는 연구자들이 관련 문헌을 반복적으로 파악하면서 중재 요소를 추출하였으며, 정기적 회의를 통해 의견 조율로 합의점을 도출하였다.

2. 분석 대상 연구 논문 및 기간

연구 문제는 ‘아동학대 중재 연구의 속성은 무엇인가?’이다. 선정기준은 (1) 연구의 대상(P)을 0–18세 아동을 대상으로 한다. (2) 중재방법(I)은 종합적 분석을 위하여 중재의 종류나 횟수, 기간 등은 배제 기준을 두지 않았다. (3)이 중 전문을 확보할 수 있는 문헌 (4) 한국어와 영어로 쓰여진 문헌만 분석에 포함하였다. 배제기준은 (1) 학대를 경험한 0-18세 아동 대상이 아닌 문헌과 (2) 아동학대 중재가 주된 내용이 아닌 문헌은 배제하였다. (3) 양육자 및 부모(비전문가)를 대상으로 한 중재 관련 문헌은 연구 목적에 부합하지 않다고 판단하여 배제하였다. (4) 단순 문헌고찰 논문은 분석에 포함하지 않았으며 연구결과가 없는 연구 프로토콜, 편집자 논평 등은 배제하였다.

3. 자료 수집

이 연구의 분석대상 연구 논문은 아동학대 중재의 목적, 이론적 틀, 학문 분야, 내용에 따라 구분하여 분석한다.
분석 대상 연구 논문의 기간은 2009년 1월 1일부터 2019년 12월 31일까지로 국한시켰는데, 이에 대한 기준은 Holt 등(2008)의 ‘The impact of exposure to domestic violence on children and young people: A review of the literature’연구가 있었기 때문이다. 이 때 검색 데이터베이스(Search Database)는 COSI 모델의 COre DB인 MEDLINE(PubMed 사용), Embase를 선택하였으며, Standard DB 중 보건의학·간호문헌의 특화 DB인 CINAHL을 선택하였다. 국내 문헌을 포함하기 위해 DBpia, 학술연구정보서비스(RISS), 한국학술정보(KISS) 검색결과를 추가하였다. 검색에 사용된 검색어는 선정한 DB별로 MEDLINE은 MeSH 검색어, Embase는 EMTREE, CINAHL은 CINAHL heading 구조를 파악하였다. 검색어의 민감도 향상을 위해 각 검색 데이터를 이용하여 검색되는 문헌의 초록을 검토한 후 유사어를 추가하여 검색어 개념도를 작성하였다. 해외 데이터 베이스에서는 문헌검색에 사용된 검색어는 다음과 같다. (1)MEDLINE(Pubmed): Child abuse(유사어: Abuse, Child/ Child Mistreatment/ Mistreatment, Child/ Child Maltreatment/ Maltreatment, Child/ Child Neglect/ Neglect, Child), Violence(유사어: Atrocities/ Assaultive Behavior/ Behavior, Assaultive), Crime Victims(유사어: Crime Victim/ Victim, Crime/ Victims, Crime/ Victimization), Therapeutics(유사어: Therapeutic/ Therapy/ Therapies/ Treatment/ Treatments), Adaptation, Psychological(유사어: Adaptation, Psychologic/ Psychologic Adaptation/ Psychological Adaptation/ Coping Behavior/ Behavior, Coping/ Behaviors, Coping/ Coping Behaviors/ Coping Skills/ Coping Skill/ Skill, Coping/ Skills, Coping/ Behavior, Adaptive/ Adaptive Behavior/ Adaptive Behaviors/ Behaviors, Adaptive), Crisis intervention(유사어: Crisis Interventions/ Intervention, Crisis/ Interventions, Crisis/ Critical Incident Stress Debriefing), (2)EMBASE: Child/Children, Abuse neglect/Maltreatment, Assault, Violence, Crime victim, Therapy, Adaptation, Crisis intervention, Management, (3)CHINAL: Child abuse/ Child neglect/ Children neglect, Violence, Crime/ Crime victims, Early intervention/Crisis intervention, Adaption, Management. 국내 데이터 베이스에서는 주제어를 {[아동] OR [청소년]} AND {[학대]OR [폭력)]} AND {[중재] OR [치료]}의 검색식을 조합하여 검색하였다. 검색 특이도를 높이기 위하여 제목과 초록, 키워드에서 검색되도록 필터링 설정하였다. 검색된 결과에서 영어와 한국어를 사용한 문헌, 원문이 확인 가능한 문헌과 인간 대상으로 제한하였다.
국외 데이터 베이스에서 검색한 결과 MEDLINE (2,994편), Embase (168편), CINAHL (293편)으로 총 3,431편, 국내 데이터 베이스에서 검색한 결과 DBpia, 학술연구정보서비스(RISS), 한국학술정보(KISS)에서 1,083편이 총 검색되어 총 4,538편의 문헌이 1차 문헌으로 추출되었다. 이중 다른 데이터 베이스에서 추출된 중복 문헌 577편을 제거한 3,961편의 문헌이 선별되었다. 이 3,961편의 논문은 연구자들이 제목과 초록을 검토하며 포함 기준에 부합하지 않는 문헌 3,909편을 제외하였다. 제목과 초록만으로 선정된 52편의 문헌을 검토한 결과 중재 연구가 아닌 경우 15편, 연구참여자가 배제기준에 해당하는 연구 9편이 추가로 제외되어 최종 28개의 문헌이 분석 대상으로 선정되었다(Fig. 1).

결 과

총 28편의 연구가 포함기준을 충족하였다. 선정된 연구의 주요한 특성은 Table 1에 제시하였다.

1. 대상 논문의 일반적 특성

이 연구에서 선정된 연구들은 2009–2019년 사이에 출판되었고, 출판 국가는 한국 16편(1–16), 미국 9편(17–25), 남아프리카(26), 독일(27), 스페인(28)이 각 1편씩 이었다. 연구설계는 단일군 사전-사후 연구가 16편(1–4, 6–9, 11, 13–14, 16–17, 20–22), 비동등성 대조군 연구가 8편(5, 10, 12, 15, 18–19, 23, 28) 무작위 대조군 연구가 3편(24–25, 27), 혼합 방법(Mixed method) 연구가 1편(26)이었다.
연구 장소는 국내외 연구 모두 주로 지역아동센터, 쉼터 및 학교에서 이루어졌다. 연구대상자 수는 1–260명이었고, 대상자의 연령대는 3–18세로 다양하였다.

2. 대상 논문에 포함된 중재의 특성

연구된 아동학대 관련 중재의 특성을 파악하기 위해 중재의 목적, 이론적 틀, 학문 분야, 중재내용에 따라 나누어 분석하였다. 중재의 목적은 심리사회적 적응과 행동 치료가 26편으로 주를 이루었으며, 그 외 1편(27)이 이후 관련 프로그램이나 서비스 참여 이용률을 목표로 삼고 있었다. 선정논문들을 학대 아동·청소년 중재를 위해 여러 이론적기틀을 근거로 사용하였는데 사용된 이론적 기틀은 반두라의 자기 효능감(self-efficacy), 트라우마 중심 인지행동 치료 모델(trauma-focused cognitive-behavior therapy model), 게임 기반 인지행동 치료 그룹 모델(Game-based cognitive-behavior therapy-group model), 대상관계 이론, 인본주의 상담 및 접촉 이론이었으며, 가장 많이 사용된 이론적 기틀은 트라우마 중심 인지행동 치료 모델이었다. 학대 아동·청소년 관련 중재를 시행한 학문 분야를 살펴보면, 심리/미술치료, 심리학, 사회복지학, 보건학 분야 등 다양한 학문분야의 참여가 이루어지고 있음을 확인할 수 있었다.

3. 중재의 내용에 따라 구분한 4가지 속성

선정된 논문들은 학대 아동·청소년의 심리사회적 적응을 돕기 위해 국내에서는 6건(1, 3, 7, 9–10, 12, 16)의 미술치료, 5건(2, 4–6, 8, 11, 13)의 놀이치료(모래상자 놀이치료 포함), 2건의 통합예술치료(놀이 및 미술 통합치료 포함) 프로그램이 있었다. 국외에서는 3건(17, 25, 28)의 인지행동치료, 1건(18)의 놀이치료, 1건(27)의 사례치료, 1건(19)의 동물치료, 3건(20–22)의 트라우마 및 놀이 기반인지행동치료, 1건(26)의 표현예술활동 치료, 1건(23)의 운동치료, 1건(24)의 임상적 치료 중심 중재가 이루어졌다. 국내보다는 국외에서 학대 아동·청소년의 심리사회적 적응을 돕기 위해서 다양한 중재가 시행되었음을 알 수 있었다. 총 28편의 중재의 요소들을 파악하여 그룹화하여 최종적으로 총 4가지 속성을 도출할 수 있었다. 추출된 중재의 속성은 대상자 중심 및 상황 중심의 학대 아동 중재, 부모-자녀간 대처능력 및 상호작용 강화, 그룹 중심과 개인 및 그룹의 통합적 중재 적용, 지역사회 기반의 중재 적용 및 다학제적 접근이다.

1) 대상자 중심 및 상황 중심의 학대 아동 중재

대상자 중심 및 상황 중심의 학대 아동 중재는 분석 대상이 된 연구의 전반에서 확인된 특성으로 대상자의 상태 및 주변과의 관계를 확인하여 이에 효과성을 나타낼 수 있는 프로그램들이 도입되었다. Kim과 Jin (2019) 연구에서는 학교폭력 피해 아동들에게는 대상자의 불안전한 심리상태를 반영하여 추후 지속적으로 영향을 미칠 수 있는 또래 및 타인과의 관계를 개선하고자 관계성 향상에 중점을 두고 중재를 시행하였으며, 가정 폭력 학대 청소년들을 대상으로 한 연구에서는 대상자의 가정환경에 대한 사전분석과 중재를 통해 대상자의 욕구를 사정하고 정서적 안정감과 불안감을 해소하고자 하는 중재(Kim & Lee, 2019)와 대상의 요구분석을 통해 요구도에 맞추어 사회성 및 행동조절 대처능력 향상, 스트레스 감소 프로그램을 적용한 중재(Jung & Cho, 2019) 등이 있다. Choi & Choi (2019)의 연구에서는 다문화 아동을 대상으로 대상자의 주요 문제인 공격성을 줄일 수 있는 인간 중심 미술치료 중재를 적용하였고, Lee (2015)는 자존감과 사회성 향상을 위한 미술치료 중재를 적용하여 자신감 및 적극성 등에 긍정적인 효과를 보고했다. Kim & Lee (2011)는 대상관계이론에 기반한 놀이치료 프로그램을 통해 공감성과 자기주장성 등 학대아동의 사회기술 향상을 확인했다. Park & Lee (2011)의 연구와 Lee (2013)에서는 모래상자놀이를 활용하여 현실적 문제에 대한 ‘중간적 공간’을 만들고 현실의 환경과 관계에 대한 통찰의 기회를 지공하여 우울감 감소를 보였다. 모래놀이치료를 통해 영웅신화론을 재구성한 이야기를 적용하여 자아를 강화시킨 사례도 있었다(Lee, 2014). Yang & Lee (2016)의 연구 및 Seo & Lee (2016)의 연구에서도 미술매체를 통한 중재 효과로 심리적 안정감의 향상을 보였다. 점진적 노출기법을 적용하여 인지행동 개인치료를 실시하여 스트레스, 우울, 또래와의 이질감, 불신감에서 향상을 확인한 연구도 있었다(Kim & Kim, 2009). 이 외에도 성적인 학대나 부적절한 성적 행동에 노출되었던 아동 및 청소년을 대상으로 한 연구에서는 게임이나 동물과 함께 하는 등의 다양한 방식의 중재를 도입하였지만, 목표는 모두 인지행동치료나 사회 심리적 적응에 중점을 두고 이루어졌다(Dietz et al., 2012; Misurell et al., 2011).

2) 부모-자녀간 대처능력 및 상호작용 강화

‘부모-자녀간 대처능력 및 상호작용 강화’의 속성을 나타내는 중재는 다음과 같다. 아동 학대를 경험한 아동만을 중재하는 단일 방향의 연구가 아니라, 부모 그룹을 함께 병렬적으로 중재하는 것이다. Hubel 등(2014)의 연구에서는 아동들의 학대 관련 낙인인식과 고립감을 개선하기 위한 중재와 함께 부모그룹이 아동들의 치료효과가 유지되고 일반화될 수있도록 아동의 행동과 감정을 이해하고 다룰 수 있도록 하는 중재가 함께 이루어졌다. Valentino 등(2019)연구에서는 학대 자녀와 어머니를 함께 중재하였는데, 이때 자녀와 함께 지난 추억을 회상하며, 아이와 대화를 촉진하기 위한 내용 등이 포함되었다.

3) 그룹 중심과 개인 및 그룹의 통합적 중재 적용

사례 연구를 제외한 다수의 연구에서 학대 아동 대상 중재 적용 시, 그룹 중심의 중재를 적용하였다. 그룹 중심의 중재나 치료는 자신과 마찬가지로 학대를 경험한 동료의 피드백과 지지를 통해 도움을 받을 수 있으므로 학대를 경험한 아동들을 위한 중재에 자주 쓰이는 방법이다(Dietz et al., 2012). 예를 들면, 성적 학대를 받은 청소년들이 그룹 중재로 인해 고립감이 해소되고 감정 표현 능력과 자기인식에 긍정적인 효과를 가져왔음을 보고한 연구가 있었으며(Allen & Hoskowitz, 2017), 폭력 및 폭력 예방방법과 관련된 주제의 그룹 토의를 병행하여 대상자의 불안, 스트레스, 대처 전략 등을 확인한 연구(Allen & Solomon, 2012)가 있었다. 또한, 성 학대 대상자의 공통적인 어려움과 관련된 활동 및 주제에 관한 세션의 그룹치료를 기본으로 하고 여기에 강아지의 참여를 추가한 한 그룹과 강아지의 참여 및 강아지가 어떠한 목적으로 그룹치료에 참여하게 되었는지에 대한 이야기를 추가한 총 3개의 그룹 치료를 시작한 후 사회 심리적 상태를 확인한 연구(Dietz et al., 2012)가 확인되었다. 또한 그룹 생활시설에서 거주하는 학대 경험 아동의 사회 심리적 상태 향상을 위해 그룹 리듬 운동을 적용한 연구가 있다(Goldshtrom et al., 2011). 이외 성적인 학대 아동 대상 중재 시 그룹 치료의 효과를 높이기 위해 Liotta 등(2015)의 연구에서는 그룹과 개별 치료 중재를 병행할 것을 제안하였다.
국내에서는 Lee & Song (2015)이 집단미술치료를 통해 아동의 우울 및 불안, 자기상에 긍정적 의미가 있음을 확인하며 관계형성에 도움이 됨을 확인했다. Kim (2013)은 집단미술치료를 통해 학대피해아동의 사회성에 대한 효과를 연구했는데, 짜증이나, 다툼, 분노감정 등의 의사 표현이 줄어들고 스스로 문제를 해결하며 경청하는 등 사회성에 효과가 있음을 보고했다. Oh & Lee (2010)는 집단놀이 치료를 통해 아동들의 문제행동이 감소되고, 특히 자아존중감의 모든 하위영역에서 점수가 향상되어 의미가 있다고 확인했다.

4) 지역사회 기반의 중재 적용 및 다학제적 접근

‘지역사회 기반의 중재 적용 및 다학제적 접근’의 속성의 나타내는 중재는 다음과 같다. Ganser 등(2017)의 연구에서는 학대 아동 및 청소년을 대상으로 사례관리자를 배정하여 초기접촉 및 상담이 이루어지고, 대상자가 적절한 치료를 받을 수 있도록 건강관리자에게 의뢰하는 단계와 치료 과정 등을 모니터링하는 단계로 이루어져 있다. Swenson 등(2010)의 연구에서는 사전 대상자 선정 시부터 신체적 학대로 인해 아동보호서비스 아래에 있는 가족들이 선정되었으며, 선정된 가족을 대상으로 다학제적인 치료 및 지역사회 기반 치료를 적용하여 사회 심리적 향상을 도모하였다. 이러한 연구들을 바탕으로 아동학대 대상자의 중재는 지역사회 환경 하에서 여러 전문가가 다학제적으로 협업하여 구현되어야 함을 입증하고 있다.

4. 대상 논문의 중재 효과

학대 아동·청소년의 중재 효과는 이후 관련 치료율 등을 본 1건(27)의 논문을 제외하고는 심리사회적 건강 및 중재 관련 지식을 종속변수로 사용하였다. 심리사회적 건강을 나타내는 변수로는 자존감/자아존중감, 정서학대/방임/표현, 대처 스트레스, 우울감/우울 행동, 사회기술평정척도(사회성, 문제행동), 협동성, 공감성, 주장성, 외상 후 스트레스 증상, 불안, 또래와의 이질감, 타인에 대한 불신감, 대처능력, 사회기술 결핍, 자기 보호 기술 등을 사용하였다. 중재 관련 지식 내용으로는 건강한 성에 대한 지식, 학대에 대한 대상자의 지식, 여러 감정 표현에 대해 대상자가 적합한 감정을 표현하는 지에 대한 지식을 사용하였다.

고 찰

매년 증가하는 아동학대의 빈도와 아동학대 유형의 복잡성의 증가는 사회의 관심을 모으고 있다. 최근 다양한 학문분야에서 학대로 인한 피해 아동의 치료와 중재와 관련한 연구가 이루어지고 있다. 이에 이 연구는 다양한 아동학대 중재 프로그램의 목적과 내용, 중재효과를 확인, 분석하고자 2009–2019년 사이에 학대 아동을 대상으로 실시되었던 국내 외 연구에 대한 통합적 고찰을 수행하였다. 결과에 따른 논의는 다음과 같다.
피학대 아동을 위해 활용가능한 중재방법으로는 인지행동치료, 미술치료, 놀이치료, 사례치료, 동물치료, 지역사회 기반 치료, 운동치료 등이 있었다. 국내 학대피해 아동을 대상으로 한 중재 연구를 메타분석한 Kim (2012)의 연구를 통해 국내에서는 주로 현실요법, 치료놀이, 무용요법, 놀이치료, 인지행동요법, 미술치료의 중재가 이루어지고 있어, 해외에서 더 다양한 중재방법을 활용하고 있음을 확인했다. 이러한 중재들은 대상자 중심 및 상황 중심의 환경 속에서 피해아동의 눈높이에 맞는 중재방법을 선택, 적용하면 아동에게 주도권을 줌으로써 학대경험으로 형성된 불안, 초조를 경감시키는 데 도움을 줄 수 있다는 기타 선행연구 결과와도 일치한다(Son & Kevin, 2003). 또 국내 연구의 메타분석 결과 자아존중감, 대인관계, 내외통제성이 주된 종속변수였는데, 이 연구에서는 여러 연구에서 그러한 변수를 학대의 표현, 대처스트레스, 협동성 및 공감성 등으로 세분화하여 측정 연구 결과를 실제 임상 적용의 가용성을 높인 것을 알 수 있었다.
아동학대는 생태체계적 관점이 연관되어 있으며, 가정 폭력은 개인의 문제가 아닌 가족 전체의 문제일 수 있다. Belsky (1980)는 생태체계적 관점에서 가족요인은 아동학대와 관련된 중요 요소임을 보고했으며, 아동학대 발생에 영향을 미치는 가족요인은 결혼만족도의 문제, 체벌허용도, 빈곤문제로(Kim & Kim, 2007) 나타났다. 본 연구에서도 여러 연구들이 가족의 일반적 특성뿐 아니라 상호 작용, 아동과 부모의 애착, 부모와 부부관계의 정도를 가족 요인에 포함시켰다. Wolfner & Gelles (1993)의 연구나 Berger (2005)의 선행연구와 마찬가지로 피해자뿐 아니라 마찬가지로 가해자도 치료가 필요할 수 있으며, 외상을 경험한 아동의 경우 가족 치료적 접근이 병행되어야 한다. 하지만 현재까지도 대부분 연구들에서 위탁양육자나 양육자를 배제한 아동만을 대상으로 한 치료 위주로 진행되고 있어, 양육자를 포함한 가족치료적 접근방법에 대한 연구가 필요해 보인다.
이 연구의 제한점은 다음과 같다. 중재의 방법과 강도(dosage) 또 그에 따른 중재 효과는 방임, 신체적 학대, 정서적 학대 등 학대의 유형과 학대에 노출된 기간, 학대의 강도에 따라 달라질 수 있다. 본 연구에서는 학대 아동을 위한 각각의 연구들에서 다양한 아동의 특성에 따른 중재 과정을 수행한 것을 확인하였다. 이 때 대상이 단일하거나 소수인 경우가 많아 주제와 관련한 연구의 대상과 방법, 결과와 중재효과 등을 일반화하기 어렵다는 점이 제한점이 있다. 피해아동의 연령, 성별, 지역을 고려하여 더 많은 사례 수를 확보한 연구를 수행한다면 외적타당도를 높일 수 있을 것이다. 또 분석의 대상이 된 총 28편의 논문연구 중 16건이 대조군이 없는 사전사후 비교를 한 중재 연구였다. 이는 연구자가 주의력 깊게 관찰하여 내적 타당도를 보장해야 하는 부분이며, 치료적 접근법을 실시한 대상과 중재를 받지 않은 대상과의 비교연구가 추후 후속 연구로 필요할 것이다.
간호사는 의료인이자 아동학대 신고의무자로서 아동학대에 대한 기본 개념뿐 아니라 다양한 현장에서 간호중재를 적용할 수 있도록 다양한 중재방법과 효과를 이해해야 할 것이다. 또 의료기관 외에도 지역사회 아동학대 관련 기관과 협력하여 심도 있는 교육과 함께 실제 임상 경험의 기회가 제공된다면, 전문성을 갖춘 담당 인력이 양성되어 아동 복지의 질에 기여할 수 있다.

결 론

이 연구에서는 2009년에서 2019년 사이에 학대·아동청소년을 대상으로 다양한 중재 프로그램이 적용된 국내외 문헌을 통합적으로 고찰하였다. 국내 16개 논문과 국외 12개의 연구를 분석한 결과 학대 아동·청소년 대상자에게 미술, 놀이, 인지행동치료, 그룹치료 등을 적용할 수 있는것을 확인할 수 있었다. 이러한 중재를 다양한 현장에서 실제로 적용하기 위해서는 중재의 내용과 원리, 적용대상 및 증상이 중요할 것이다.
학대 아동·청소년을 대상으로 한 중재프로그램을 살펴봄으로써 적극적 개입을 통해 피해 아동 청소년의 사회심리적 건강 및 학대 관련 지식이 긍정적인 방향으로 변화하는 것을 확인할 수 있었다. 따라서 피해 아동·청소년을 위해 조기개입 및 적극적 개입을 통한 치료 및 중재를 시행하는 것이 중요하며 이는 아동 개인의 지속적인 삶을 위해서일 뿐 아니라 지역사회의 건강 증진을 위해서도 의미가 있는 대안이라고 할 수 있다.
이 연구는 학대 아동·청소년을 대상으로 한 프로그램의 목표, 중재방법, 내용, 중재 효과를 고찰하였다. 또한 앞으로도 지역사회나 병원 등 현장에서 학대로 인한 피해아동을 대하고 중재해야 할 때, 본 연구를 통해 피해아동을 위한 치료 및 중재의 방법과 내용을 제공하는 데 그 의의가 있다.
