Noh, Lim, Kim, and Chin: Predictability of pancreas invasion from gastric cancer by CT

Abstract

CT is advocated as a staging tool in hope to avoid exploratory laparotomy in patients with gastric carcinoma,but CT often fails to predict invasion of pancreas and splenic hilum. We studied a series of 128 patients withgastric adenocarcinoma who underwent CT and surgical exploration, to evaluate the predictability of pancreasinvasion by CT. CT accurately predicted the preservation of pancreas in 93 patients, while CT failed todemonstrate the pancreas invasion in 23 patients. Spread to pancreas was correctly predicted in 7 patients out of30 patients, and 5 patients lacking fat plane had no invasion, whereas 10 patients with an intact fat plane hadinvasion. Sensitivity was 23.3%, specificity was 93% and overall accuracy was approximately 78.1%.
