Jeon, Lee, Kim, Park, Park, Song, Kang, Lee, and Park: Primary Invasive Carcinoma of the Vulva: Twenty-two Cases

Abstract

Twenty two patients with primary invasive carcinoma of the vulva were treated at Seoul National Univeraity oapital from 1980 to 1991 and their clinieopathological pr ofilea were analyzed retrcepectively. The medien age ef the patients was 50 years(32-81 years). The most frequent preseniting symptorm was a palpable lump or mass observed in 14 patients(63.6%), followed by pruritia in 12 patients(54.5%) and pain in 10 patients(45.5%). As a standard therapy, radical vulvectomy wiith bileteal inguinofemoral lymphadenectomy was performed in 17 patienta and simple vulveatomy waa done in one patient. Eight patients of them had reconstvuctive surgery with loeal cutmneoue(5 patients) or myocutaneous(3 patients) flaps. Chemo-therapy or radiotherapy wae performed in 3 patients and one patient rejeeted the treatment. Nineteen patientas(86.4%) had aquamous cell caxcmoma, 2(9.1%) had adenocarcinoma of the BarthoIine gland, and one(4.5%) had melanama. Of 18 surgically treated patients, 5 were stage I (27.8%), 7 were sta,ge 11(38.8%), 2 were stage III(11.1%), 3 were stage Wa(16.7%) according to FiGO aurgical staging, and one had melanoma of Clarks level IV. Two(11.1%) of them had lymph node metastasia There was no caw of primary mortality but morbidity wae obeerved in 16 patients(88.9%). The 5 year survival rate of 18 surgically treat,ed patients was 65.3%(median follow-up 47 months). This experience suggests that more conservaiive surgery should be considered in the rnana rnent of selected patients with primary invasive vulvar cancer.
