Kim, Whang, and Song: Treatment of CDH in Walking Age

Abstract

The patients of congenital dislocation of the hip are almost found after the walking age because of no national screening system in this country. But the choice of treatment are still in a controversy between the conservation and operative method. This study is a report of the results of 27 patients (29 cases) in congenital dislocation of the hip who have visited our hospital at the age of 1 to 3 years old, and have been followed more than 12 months (14-69 months) on the review of the chart, plane film and arthrogram. The results were as follows;1. The age at diagnosis was 20.3 months (14-35 months) in average. The method of treatment were consevative for 12 patients, and operation for 9 patients and secondary operation for 6 patients who were failed with consevative therapy. The interval between consevative therapy and operation was 13.5 months (12-15 months). 2. A-E ratio which was checked on the arthrogram was 0.81 in consevative group, 0.74 in operative group and 0.75 in secondary operation group. 3. By Severin's radiologic grade, the result was good in 7 cases, fair in 3 cases and poor in 2 cases in consevative group. In operative group, good was in 5 cases, and fair in 4 cases. The patients who were converted to operation were good in all cases. 4. It might be suggested to be consevative therapy for the patient with over 0.8 of A-E ratio in early walking age group.
