Kim, Lee, and Ahn: Treatment of Open Fractures with Internal Fixation

Abstract

56 cases of open fractures were treated with internal fixation at Knagnam General Hospital from Mar. 1982 to Dec. 1989 and all of them were followed up more than one year. By Gustilo classification, 9 cases were Grade I, 30 cases were Grade II, 9 cases were Grade IIIa, and 8 cases were Grade IIIb. We have treated all these patients with internal fixation devices; plate and screws 27 cases, Ender nailing 16 cases and Kuntscher nailing 7 cases. Implants were used to achieve rigid fixation and wounds were not closed until the signs of inflammation disapperared. All fractures except one were united (union rate; 98.2%) and postoperative infection rate was 14.2% (8/56 cases). As Chapmann have said, we believe that internal fixation can be one of the solutions even in Grade II, III open fractures.
