Park and Choi: Bipolar Hemiarthroplasty for the Treatment of Femoral Neck and Unstable Intertrochanteric Fracture in Elderly Patients

Abstract

Bipolar hip prosthesis was developed to alleviate the problems of unipolar prosthesis, including joint space narrowing, pain, and acetabular protrusion but the indications for it are still in an evolutional phase. From June, 1987 to August, 1990, 90 Bipolar hemiarthroplasty was done at Dept. of Orthop. Chonbuk National University Hospital for the displaced femoral neck fracture and unstable intertrochanteric fracture, and among them 60 cases were followed up more than 12 months in 30 femoral neck fractures and 30 intertrochanteric fractures. The followings are the results: l. Among 60 cases, most patients were 65 yrs old and have displaced femoral neck or unstable intertrochanteric fracture. The functional results assessed by DAubigne and Postel scale, were rated good. 2. The complications were intraoperative femoral fracture and superficial wound infection, etc. Acetabular protrusion has not been developed so far. 3. In elderly patients, rehabilitation was easier and faster following bipolar prosthesis than internal fixation. The incidence of pressure sore and pulmonary complication was lower in bipolar prosthesis. The possibility of early walking with full weight bearing after bipolar prosthesis is considered to be major contributing factor to these results.
