Kim, Rhee, Kim, and Lee: Geographic Double V-Osteotomy for the Correction of Angular Deformity of Distal Humerus

Abstract

There are so many ways to correct the angular deformities of distal humerus. The double V-wedge out osteotomy to correct the angular deformity as well as the mild rotational deformity, too are very effective to achieve stability on osteotomy site and to get the rapid bony union followed by early ROM exercise. Since 1980, we have performed the total 14 cases of double V-wedge osteotomy and the results obtained were as follows. l. We devised a new method to estimate the rotational deformity of distal humerus using C-arm image intensifier. 2. The indication of this operation was in the rotational deformity with less than 20°3. Eleven cases had cubitus varus with average 17.8°and 3 cases were cubitus valgus with average 44.7°of carrying angle, their average age were 15.5 years old and the time elapsed from initial trauma to operation were average 13.4 years. 4. After the double V-wedge out osteotomy, the distal humerus was fixed with 2-3 K-wires and immobilized with sugar-tong cast for 6 weeks followed by active gentle ROM exercise. 5. Post-operatively the carrying angle was improved to average 13.1°in cubitus varus and 11°in cubitus valgus cases, and the normal ROM of elbow was gained after follows-up for average 28 months. But delayed union was found in one case due to distraction gap at osteotomy site.
