Park, Han, and Byun: Clinical Study for Lateral Condyle Fracture of Humerus in Children

Abstract

Twenty-three patients with lateral condyle fracture of humerus were treated at department of orthopedic surgery, Hae Hwa hosp, Medical center, Korea University from March 1984 to June 1989. They were followed up from one year to four years and eight months with an average of 16.7 months. The authors analysed the methods of treatment on the bases in the degree of displacement, Change of carrying angle and complication. The summerized results were as follows;1. The age incidence was confined from age of 2 to age of 13. Mean age at time of fracture was 6.3 years. 2. Most of fractures were Milch's type II (20 cases). There were 5 cases of Jakob's stage I, II cases of stage II and 7 cases of stage III. 3. In 9 casese of closed reduction and long arm cast immobilization, Changes of carrying in fractured side were between minimum decrease of 8 degree and maxmum increase of 9 degree in compair with normal side and also minimum decrease of 10 degree and maximum increase of 9 degree in 3 cases of closed reduction and percutaneous K-wire fixation. In 11 cases of open reduction, Changese of 5 degree and maximum increase of 2 degree in compair with normal one. 4. The change of carrying angle according to initial displacement of fracture site, there were minimum decrease of 5 degree and maximum increase of 2 degree in stage I, Minimum degrease of 10 degree and maximum increase of 9 degree in stage II and also minimum decrease of 5 degree and maimum increase of 9 degree in stage III. 5. Four lateral condyle overgrowth, One delayed union and one avascular necrosis were observed as complications, But clinically significant complications were not noted. 6. Above results suggest that open reduction and internal fixation in cases of over 3 mm displacement or rotation of fragment is necessary for lateral condyle fracture of the humerus in children.e fracture of the humerus inchildren.
