Lee, Kwon, and Koo: Treatment of Unstable, Comminuted Intertrochanteric Fracture of the Femur Over 60 yrs. of Age with Ender Nails

Abstract

The unstable, comminuted intertrochanteric fracture of the femur in elderly is a serious and difficult problem, especially, internal fixation is a much greater problem and secondary displacement or implant failures with subsequent re-operations occur rather frequently. There seem to be different opinions about what kind of internal fixation device is preferable. Ender nailing has been used in intertrchanteric fractures of the elderly patients. Ender nailing has the following advantages;A low infection rate, a small demand for blood transfusion, early mobilization with full weightbearing, reduced hospitalization time and reduced mortality. The authors analyzed twenty-five cases with more than six months follow-up of unstable, comminuted intertrochteric fracture treated with Ender nails during the period from 1987 to 1990 and the results are as follows:1. The ratio between men and women was 1:1.3 and the average age was 70.8 years. 2. The most common causes were the slip down injuries. 3. The most common one was type 3 according to the Tronzo, et al(19,20) classification. 4. According to the Singh's index, the poor in bone quality were 15 cases. 5. Intraoperative complications occurred in 7 cases and postoperative complications occured in 8 cases which included range of motion limitation of knee joint in 5 cases, proximal migration of nails in one and distal migration of nails in two. 6. The average bony union time was about 15.4 weeks.
