Kim and Shin: Reverse Forearm Flap in Treatment of the Scar Contracture in Hand

Abstract

Scar contracture of the hand after burn or crushing injury is cosmetically unacceptable and sometimes result in limitation of motion of the hand. We can treat by Z-plasty or local flap when the area of scar contracture is not large. But if the area of scar contracture is extensive, coverage of soft tissue defect after excision of scar tissue is not a simple problem and there are several methods of the soft tissue coverage such as distant flap or free vasculized flap. The reverse forearm flap is one of the methods of the soft tissue coverage and is a fasciocutaneous flap based on the radial artery and its venae comitantes. The authors had treated 8 cases of extensive scar contracture of the hand after burn or crushing injury by excision of scar tissue and reconstruction with reverse forearm flap. The results of these operation were satisfactory and we recommand that the reverse forearm flap is one of the safest, simple and effective one-stage soft tissue reconstruction for the soft tissue defect in the hand.
