Bae, Yoo, and Kim: Comparative Analysis of Cemented Versus Cementless PCA Knee

Abstract

Between April 1984 and February 1987, eighteen patients (25 knees) with osteoarthritis or rheumatoid arthritis were treated at the department of orthopedic surgery of Kyung Hee University by porous coated anatomic knee. Among them, 16 patients (23 knees) who could be followed for more than 3 years were analyzed. In the analysis, we compared the result of cemented TKR patients (group I ) with those of cementless TKR patients (group II ). The number of cases were 17 knees (11 patients) in group I and 6 knees (5 patiets) in group II. The average age of the patients was 59.8 years in group I and 56.4 years in group II. The average follow-up period was 5 years and 1 month in group I and 4 years and 1 month in group II. The Knee Rating Scale of Hospital for Special Surgery was used for the clinical assessments and the American Knee Scoiety Roentgenographic Evaluation and Scoring System was used for the radiological assessments. The results of analysis are as follows;1. The ROM of knee was 115°preoperatively and 106.3°postoperatively in group I and 111.7°and 119.3°in group II. 2. Flexion contracture was markedly decreased in both group. (Preoperative average of flexion contracture was 15.8°in group I and 15°1 in group II ). 3. Preoperative T-F angle was varus 4°in group I and group II. Postoperatiely, it was corrected as valgus 4.3°in group I and valgus 1.5°in group II. And the tibial component insertion angle was varus 4°in group I and varus 3.7°in group II. 4. The knee rating score improved from 56.1 to 87.4 points in group I and from 55.5 to 86.5 points in group II. 5. Ten of 23 knees had radiolucent lines at the bone-cement or bone-proshtesis interfaces that were noticed in 8 cases of group I and 2 cases of group II. But, except one case in group II, all of the nine cases were nonprogressive and insignificant clinically. 6. There was one case of postoperative infection in group I and it was proved as tuberculosis with culture. And the infection was controlled after debridement with antituberculous chemotherapy.
