Kim, Park, Kim, and Choi: Resection Arthrodesis for Giant Cell Tumor in Distal Femur: Two cases Using Vascularized and Neovascularized Fibular(Dual Fibular) Graft

Abstract

In the management of giant cell tumor with the tendency to recur or metastasize, several things should be considered such as location and condition of lesion. The ideal aim in the management of giant cell tumor is to eradicate the tumor and still save the joint. When giant cell tumors occur in expendable bones, en bloc resection is the treatment of choice. En bloc resection of major joint requires a facility with reconstruction technique including the use of allografts, large autogenous graft and fusion, or custom-made prosthesis. But, by conventional large bone graft, it is difficult to achieve bone union and several complications could be occurred because of prolonged immobilization of the limb. Also custom endoprosthetic replacement have a problem such as an expansiveness and loosening and mechanical failure. Two cases who were treated with a vascularlized fibular graft and a conventional bone graft with iliac bone and another fibula after tumor resection of the giant cell tumor in distal femur in Bucheon's Holy Family Hospital are reported.
