Abstract
In the recent, the advent of more specific diagnostic tools, the widespread availability of whole blood and volume expanders, the possibility of direct and early surgical intervention, and the continued assessment and analysis of therapies in clinical and experimental environments have contributed in a major way to the improved prognosis and decreased mortality rates for fractures of the pelvis. In the unstable fractures of the pelvis, surgical intervention have been tried with development of the external and internal fixators and with improvement of the surgical approaches, but most cases of the pelvic bone fracture were stable and good results by conservative treatment have been reported. The authors reviewed 265 cases of the pelvic bone fractures treated at the Orthopaedic Department of Hanyang University Hospital during the period from June, 1983 to June 1989. And Among 265 cases, with the exception of the children before 20 years old and single acetabular fracture, 196 cases, that could be followed up were analysed clinically and radiologically. Following results were obtained. l. Of the 196 cases of the pelvic bone fractures, unstable Type III fractures were in 64 cases(32.6%), Type I-B in 35 cases, Type II-A in 67 cases and Malgaigne fractures in 35 cases were most common among the Type III fractures. 2. In the stable pelive bone fracture, the almost cases by treated with conservative treatment showed satisfactory results. 3. The retroperitoneal bleeding and injuries of genitourinary tract were most common associated soft tissue injuries and showed high incidence in Type III fractures. 4. Of the 196 cases, good results were 137 cases(71.7%). But in Type III fractures poor results were 18 cases(30.5%) of the 61 cases except death(3 cases) and mainly contributed to the poor result in the whole pelvic bone fractures. 5. Although we had performed operative treatment in only 6 cases and not achieved final results, it is suggested that the operative treatment with external or internal fixators as aggressive method could be considered to be a method of treatment in severe unstable fracture like as Type III, for early stabilization of the pelvic ring, improved recovery of associated injureis, and early mobilization of the patients.
