Park and Kim: Ipsilateral Femoral Neck and Shaft Fracture

Abstract

Ipsilateral femoral neck and shaft fractures show the diagnostic difficulty and the complexity of the treatment. This double fractures, when, not, initially diagnosed, carries a much high risk of complications, such as, aseptic necrosis of the femoral head. At the department of Orthopaedic Surgery, Chonbuk National University, from May, 1986 to March, 1991, twelve cases of the concomitant ipsilateral femoral neck and shaft fractures had been treated. The results are as follows;1. The cause of the fracture is a traffic accident and there were, mostly, in men beween 30 and 50 years old. 2. Ths site of the femoral neck and shaft fracture was, chiefly, basicervical and midshaft. Most of the associated injuries were tibial fractures. After, the reduction of the shaft fractures, to reduce the neck fracture is convenient for the surgeon. Recently, we achieved the good results with the reconstructive I-M nailing. But the long term follow-up is anticipated. 3. The complciations were the aseptic necrosis of the femoral head, nonunion and the superficial wound infection. 4. The hip fracture must be confirmed on the femoral shaft fracture, and then, adequate procdure mus be selected, dependent on the age of the patients, the fracture site and the degree of the comminution.
