Cha, Lee, Kim, and Lee: The Treatment of Intertrochanteric Fracture using a Sliding nail Plate

Abstract

The primary goal in the treatment of an elderly patient with an intertrochanteric fracture is to return the patient to his prefracture activity as soon as possible. For these reason, treatment of intertrochanteric fractures by reduction and internal fixation has become the standard procedure. The choice of internal fixation device is influenced by the general condition of the patients, the pattern of fracture and the personal preference of surgeon. There are many reports that sliding nail plate appears to give better results than fixed nail plate because the latter device leads to high failure rate. Authors treated 54 intertrochanteric fracture by using sliding nail plate, from March 1985 to March 1990. The result are as follows:1. The causes of injury under 50 years old were fall down and traffic accident. 2. Of 54 case of intertrochanteric fracture, 39 cases(72.2%) were unstable fracture. 3. The average length of sliding of the lag screw is 7.5mm. 4. The average bone union time was about 15.2 weeks. 5. We experienced 7 case complications, more common in low trabeular grade group and in unstable fracture. 6. We considered that the risk of technical complication is dependent on the degree of fracture comminution and the degree of osteoporosis.
