Kim, Kim, Jin, and Moon: Clinical Study of Kim's Femoral Stem

Abstract

Repalcement arthroplasties were performed for 26 hips in 24 patients with Kim's femoral stem from June to September, 1988. This study includes 22 hips in 21 patients who were followed up at least for 2 years. Fifteen patients (15 hips) were males and 6 were females (7 hips). Age ranged from 21 years to 67 years, on an average 45 years. The causes were avascular necrosis in 16 hips, primary osteoarthritis in 2, infection sequelae in 2 and others in 1. Seven hips had bipolar endoprosthesis and 15 total hip replacement arthroplasties. Bone cement was not used in 20 hips, among which 2 were with bone graft. Aesculap cups were used in 13 hips and DePuy and Mecron cups in the others and ceramic heads of Johnson and Johnson were applied in 12 hips and Aesculap, Mecron and Osteonics for the remained. DePuy cups and DePuy, Johnson & Johnson and Aesculap heads were used in bipolar endoprosthesis. The average preoperative Harris score was 49.9 and increased to 96.2 at the last follow-up. Flexion contracture was improved from 17.5°to 1.0°, further flexion from 102°to 119°, external rotation from 25.8°to 57.5°, internal rotation from 8.5°to 23.6°and adbuction from 37.5°to 62.4°. Intraoperative complications were linear crack at the femur shaft in 3 hips and calcar crack in one. Postoperative complications were painful trochanteric wire in one case, heterotopic ossification in one, trochanteric nonunion in one and dislocation in one which had subnormal proprioception. There were two cases with radiolucent line around the stem without any pain. Neither calcar resorption nor subsidence was observed on roentgenogram.
