Abstract
A wide variety of nonoperative and operative methods are currently available for the management of humerus shaft fracture. Authors experienced the fracture of humeral shaft in 28 cases who were treated from May 1989 until May 1991. In this paper authors report the result of 19 cases with minimum follow up of 12 months after operation. Among them, 14 cases were fresh fractures and 5 cases were old nonunions due to metal failure. The results were as follows. 1. The age distribution were from 23 years to 83 years(average 39.3 years) and male patients were 13 cases and female patients were 6 cases. 2. The most common type was transverse fracture(10 cases, 50%) and the most common site was middle one third(10 cases, 50%) next was distal one third(5 cases, 30%) and proximal one third(4 cases, 20%) in order. 3. Among 14 cases of fresh fractures, closed nailing was done in 8 cases, closed nailing with interlocking screw fixation in 4 cases and open nailing with wiring in 2 cases of communited fractures. Five cases of old nonunions were treated by open nailing with bone graft. 4. The average time of bone union were about 7.5 weeks in fresh fractures and 9 weeks in old nonunions. 5. In all cases, the average range of motion of shoulder were 165 degrees of abduction, 45 degrees of external rotation and 50 degrees of internal rotation with satisfactory funtional outcome. But two cases of bursitis and skin irritation of protrued proximal Kuntscher nail tip occurred with slight impairment of abduction. 6. Authors obtained satisfactory results after Kuntscher intramedullary nailing, and conclude that it can be performed safely and effectively in fracture of the humeral shaft.
