Yang, Choi, Chung, Kim, and Kim: Pedicular Screw Fixation of the Thoracolumbar Fracture using Cotrel-Dubousset Instrumentation

Abstract

Authors have treated 19 cases of the injuries of the thoracolumber spine (T11 to L2) by short segment pedicular screw fixation using C-D instrumentation during the period from June. 1990 to Sept. 1991. The results obtained were as follows; 1. Corrections after short segment fixation was as satisfying as follows: Ant. wedging angle, 8.4°; Local kyphosis, 12.2°; Ant. vertebral height, 28.4%; Post, vertebral height, 3.6%. 2. Maintaing correction appeared to be satisfying as well. Correction loss was as follows : Ant. wedging angle, 2.0°; Local kyphosis, 2.8°; Ant. vertebral height, 4.4%; Post. vertebral height, 0.6%. 3. In 11 cases of out of 13, level of neurologic injury even without performing laminectomy was improved by average 1.1 grade. 4. Short segment fixation was effective in correction, its maintenace, and recovery of neurological function in the following cases; Compression fracture with neurological injury or above 50% compression; Bursting fracture with below 30% neural canal encroachment; Chance fracture; Fracture-Dislocation with minimal bony fracture.
