Abstract
Between July 1986 and June 1990, 115 Miller-Galante total knee arthroplasties were performed in 82 patients. Among them, 112 knees in 79 patients which could be followed more than one year were analyzed. At the analysis, we compared the result of cemented TKR patients (group I) with those of cementless TKR patients (group II). The number of cases were 59 knees (40 pts) in group I and 53 knees (39 pts) in group II. Their average age was 53 years in group I and 55.7 years in group II. The average follow-up period was two years & three months in group I and two years & 10 months in group II. There was no significant difference in both groups. The knee rating scale of Hospital for Special Surgery was used for the clinical assessments and the American Knee Society roentgenographic evaluation & scoring system was used for the radiological assessments. The results of analysis are as follows; 1. The ROM of knee was 87.6° preoperatively & 110.2° postoperatively in group I, and 98.3° and 112.2° in group II. 2. Preoperative flexion contracture was 11.5° in group I and 24.3° in group II, but it was decreased markedly in both groups. 3. Preoperative TFA was varus 6.7° in group I and varus 5.4° in group II. Postoperatively, it was corrected as valgus 4.3° in group I and valgus 5.2° in group II. 4. The Knee Rating Scale improved from 50.7 to 87.7 points in groupI and 56.6 to 94.8 points in group II. 5. Radiolucent zones with less than 2mm were noticed in five knees of group I & eight knees of group II, but those were nonprogressive and insignificant clinically. Among the eight knees of group II, sclerotic lines & radiolucencies were present in five knees around the fixation screws. It was thought that micromotion occured in the early period after implantation. The patellar malalignments (subluxation, tilting etc.) were noticed in four knees of group I & five kness of group II. 6. Among group I, deep infection occured in one knee two years after replacement surgery. And a good range of motion obtained after two staged revision procedure. Conclusively, according to the clinical & radiological assessments, there were no significant differences in both groups.
