The incidence of the intertrochanteric fracture of the femur is increasing because of enlargement of geriatric population. The primary goal of the treatment in elderly patient who was have an intertrochanteric fracture is stabilization of fracture site and firm intemal fixation for the rapid ambulation, decreasing mortality and restoration of function. Many devices were developed for this purpose, especially compression hip screw had gained considerable acceptance. A retrospective study of roentgenograms and medical records of the 39 patients who was have intertrochanteric fracture was done from March 1990 to October 1991. The average follow up duration was 16 months (range; 12 months to 28 months). The summaries of the results were as follows; 1) The mean age was 70.5 years old and male to female ratio was 1: 1. 2) In the 20 cases which are anatomical bent plates (intraoperatively bent dynamic-compression plates and Rowe's plates, Group I) were used, the patients' average age was 72 years old and in the 19 cases which are compression hip screw (Group II) were used, the patients average age were 68.8 years old. 3) The stable fractures were 36% and the unstable fractures were 64% by Evans classification. 4) The osteoporotic patients were 55% and they had high ratio of unstable fractures. 5) The mean operation time was 75 minutes in the Group I, and 95 minutes in the Group II. 6) Bony union was obtained 17.3 weeks in the Group I, and 15.1 weeks in the Grou II. 7) The ratio of complicated cases was 25% in the Group I, and 21% in the Group II. 8) In the stable fractures and the nonosteoporotic cases, the Group I had a similar results to the Group II, but in the unstable fractures and the osteoporotic patients, the Group I had the poor result to Group II. 9) If the indications are chosen well, the anatomical bent plates will result in a good outcome.
