Lim, Lee, Kim, Kim, and Shim: A clinical study of revision total hip arthroplasty

Abstract

Since the total hip arthroplasty was documented by Chanley in 1960, as the rapid development of the biomechanics, metalology, and the surgical technique, the indication of THA are getting broader. The increase in the number of revision is the result of longer follow up and frequent indentification of complications like infection and aseptic loosening. We have experienced 29 hips of revision, in 29 patients, from Feb. 1984 to Feb. 1991. The longest follow up was 7 years and 1 month ahd the shortest was 1 year and 4 months, the average being 4 years and 8 months. 1. The main cause of revision surgery was aseptic loosening (81%), and revision site were as follows: acetabular component 7 cases (24%), femoral component 6 cases (27%), both components 16 cases (55%). 2. The time interval between primary operation and revision arthroplasty was average 7 years and 1 month. 3. The most common prosthesis used in primary THA was Charnley-Muller type in 12 cases (41%). 4. The clinical results assessed by Harris hip score system were improved from preoperatively average 60 points to average 79 points postoperatively. 5. The complications were as follows: femoral fracture 5 cases, dislocation 3 cases, aseptic loosening 3 cases, trochanteric nonunion 2 cases, and uncontrolled infection 1 case. 6. Rerevision total hip arthroplasty was done in 4 cases.
