Twenty-six hips in twenty four children with neglected developmental dislocations of the hip were surgically treated by authors. The surgical regimen for these hips were i) open reduction plus femoral shortening (9 hips), ii) open reduction plus innominate osteotomy (7 hips) and iii) open reduction plus concurrent femoral shortening and innominate osteotomy (10 hips). This paper present the outcomes of the procedure at an average follow-up period of 5 years 6 months (ranged from 1 year 2 months to 10 years 5 months). In all hips except two, the late radiological results were either grade I or grade II according to the Gibson and Bensons modification of Severins classification. The clinical results, however, were better than the radiological results. Eight hips belonged to clinical group I and X-ray grade I (30.8%), two hips to clinical group II and X-ray grade I (7.7%), nine hips to clinical group I and X-ray grade II (34.6%), four hips to clinical group II and X-ray grade II (15.4%), one hip to clinical group I and X-ray grade III (3.8%), one hip to clinical group III and X-ray grade III (3.8%). The complications included partial ischemic necrosis of the femoral head in two hips and femoral shaft fractures during physical therapy in one hip.
