Shin, Jung, Yi, Kim, and Cho: Clinical Observation for the Treamtnent of Talus Fracture

Abstract

Talus fracture is rare and so surgeons experiences in treatment of the talus fracture is limited. More than 60% of the surface of the talus is articular and unless its various joint have near-full range of motion, normal gait mechanism will be impossible. Authors clinically analyzed 25 cases of fracture of the talus, treated at Haesung hospital from November 1984 to December 1990. The results were as follows; 1. Of the 25 cases, there were 22 males and 3 females, and the average age was 32 years, ranged form 21 years to 58 years. 2. The most common cause was falls from height (52%), and violent hyperdorsiflexion of the ankle was common mechanism of the fracture. 3. There were 12 neck-fractures (according to classification by Hawkins, 5 cases were type I; 3 were type II; 4 were type III), 6 body-fractures, 5 process-fractures, one head-fracture and one subtalar fracture-dislocation. 4. Complications were avascular necrosis in 4 cases, posttraumatic arthritis in 4, and skin necrosis in 1. 5. Final results, evaluated by Hawkins scoring system, were as follows: excellent in 12 cases, good in 8, fair in 4, and poor in l. 6. Satisfactory results were obtained in 3 of 4 patient with avascular necrosis treated conservatively, but clinical results of a patient with avascular necrosis treated by late arthrodesis was poor.
