Yoon, Jeon, Oh, Jung, Kim, and Lee: A Clinical Study on Fracture Dislocation of the Talus

Abstract

Fractures of the talus are uncommon injury, but they are considered as difficult fractures because of the poor outcome after treatment. Anatomically, talar sufrace is covered mainly with articular cartilage and blood supply to the talus is very poor. Therefore complications, such as non-union, avascular necrosis, and traumatic arthritis, are frequently encountered. Authors have analysed 14 cases of the talar fractures treated at Sung-Ae General Hospital from January 1,1987 to December 31,1990 with minimal 1 year follow-up period and have obtained following results: l. Of 14 cases, 13 male and 1 female, average age was 27.1 years. 2. Traffic accident was the most common cause of injury (7/14). 3. 5 ipsilateral medial malleolar fractures, 1 ipsilateral tibiofibular fracutre, 1 contralateral femur fracture and 1 compression fracture of the spine were associated. 4. According to the Hawkins classification 6 cases in type I, 4 cases in type II, 4 cases in type III fractures were observed. 5. Excellent results were observed in 7 cases, good in 4 cases, fair in 3 cases with the Hawkins therapeutic criteria. 6. 3 ankle stiffness, 2 posttraumatic arthritis, 1 skin necrosis, 1 avascular necorsis, were observed as complications but nonunion was not observed. 7. MRI was useful in diagnosis of the avascular necrosis.
