Cho, Kim, Yu, Yang, and Kim: Surgical Treatment of Spinal stenosis

Abstract

Spinal stenosis has several types of etiology such as degenerative, congenital, spondylolisthetic and postoperative, etc. Operative management for spinal stenosis is adequate decompression and stabilization of the unstable lumbar spine created by the decompression procedure. We analysed 109 operative cases of spinal stenosis who were admitted at National Medical Center from July 1981 to July 1990. The following results were obtained; 1. The male and female ration was 1.1: 1 and mean follow-up period was 6 years and 4 months. 2. The involved site was 1 level in 29.4%, 2 levels in 58.7% and more than 3 levels in 11.9%. 3. The types of spinal stenosis were showed lateral recess type in 34.9% and mixed type in 65.1%. 4. For decompression method, total laminectomy combined with foraminotomy and facetectomy was the most frequently employed procedure (88.1%). 5. For stabilization, bilateral lateral or posterolateral fusion was performed in major damage of facet joint, multiple level decompression and combined degenerative spondylolisthesis. Instrumentation was combined in 30.3% and not combined in 30.3% of fused cases. 6. According to the criteria of Kirkaldy-Willis, Excellent and good results were shown in 81% of instrumented group, 73% of fused group without instrument and 77% non-fused group and 77% in overall.
