정 and Jeong: 인크레틴 치료의 심혈관질환 안전성에 대한 연구

Abstract

In 2008, the United States Food and Drug Administration issued guidance which mandated long-term cardiovascular outcome trials (CVOTs) to assess the safety of new antidiabetic drugs for type 2 diabetes. Since 2008, three CVOTs that have studied dipeptidyl peptidase-4 (DPP-4) inhibitors and four CVOTs of a glucagon-like peptide-1 (GLP-1) receptor agonist (GLP-1RA) have been reported. Each of the completed CVOTs showed the noninferiority of respective drugs to placebo for primary CV composite endpoint. Among them, liraglutide and semaglutide showed a reduction of major adverse cardiovascular events. However, the mechanisms for the observed cardiovascular differences between DPP-4 inhibitors and GLP-1RA, and across individual GLP-1RA are not clearly understood. Therefore, this review will summarize the CVOTs of the DPP-4 inhibitors and GLP-1RA, interpretation of cardioprotective results of incretin-based therapy and the possible mechanism of action.

서론

심혈관질환은 당뇨병 환자의 사망률과 이환율을 증가시키는 주요 원인 질환이다. 당뇨병 환자의 심혈관질환을 예방 또는 지연시키기 위해서는 혈당 조절뿐 아니라 이상지혈증, 고혈압, 흡연과 같은 심혈관질환의 위험인자에 대한 관리가 중요하다.1 특히 혈당조절에 있어서는 엄격한 혈당 조절이 모든 당뇨병 환자에게 적합한 것은 아니기 때문에, 환자의 혈관합병증이나 동반질환 유무, 당뇨병의 유병기간, 저혈당 대처능력, 가족의 지지 등을 고려하여 개별화된 혈당 조절 목표를 설정하고 환자 개개인에 적절한 약제를 선택하는 것이 권고되고 있다.2
1990년 이후 많은 당뇨병 약제가 출시되면서 혈당 조절에 대한 효과 이외에도 안전성에 대한 검증이 중요해졌다. 그러던 중 2007년에 Nissen 박사에 의해 thiazolidinediones 계열의 당뇨병 약제 중 rosiglitazone 이라는 약제가 심근경색과 같은 심혈관질환을 증가시킨다는 메타분석이 보고된 이후,3 2009년에 미국의 Food and Drug Administration (FDA)은 rosiglitazone 을 판매 철회시켰으며, 그 후 나오는 모든 새로운 당뇨병 약제는 심혈관질환의 안전성을 입증해야 한다고 하였다. 그래서 그 후 출시된 dipeptidyl peptidase-4 (DPP-4) 억제제나 glucagon-like peptide-1 (GLP-1) 수용체작용제의 인크레틴(incretin) 약제와 sodium glucose cotransport-2 (SGLT2) 억제제의 심혈관질환에 대한 대규모 임상 연구들이 진행되어 최근 일부 결과들이 발표되고 있다.
DPP-4 억제제는 지금까지 3개의 연구 즉, Saxagliptin Assessment of Vascular Outcomes Recorded in Patients with Diabetes Mellitus-Thrombolysis in Myocardial Infarction 53 (SAVOR-TIMI 53),4 Examination of Cardiovascular Outcomes with Alogliptin versus Standard of Care (EXAMINE),5 Trial Evaluating Cardiovascular Outcomes with Sitagliptin (TECOS)6 연구가 발표되었고, GLP-1 수용체작용제는 4개의 연구 즉, Evaluation of Lixisenatide in Acute Coronary Syndrome (ELIXA),7 the Liraglutide Effect and Action in Diabetes: Evaluation of Cardiovascular Outcome Results (LEADER),8 Evaluate Cardiovascular and Other Long-term Outcomes with Semaglutide in Subjects with Type 2 Diabetes (SUSTAIN-6),9 Exenatide Study of Cardiovascular Event Lowering study (EXSCEL)10 연구 결과가 발표되었다(Fig. 1). 이 연구들을 통해 인크레틴 제제의 약물은 당뇨병 환자에서 심혈관질환을 더 일으키지 않는 안전한 약제임이 밝혀졌고, liraglutide나 semaglutide는 오히려 심혈관질환을 감소시킴이 확인되었다(Fig. 2). 이런 효과는 혈당강하 효과 이외에 인크레틴에 의한 혈압감소, 체중감소, 이상지질혈증 개선 작용과 GLP-1의 혈관에 대한 직접적인 작용이 있으리라 생각된다.

따라서 이 논문에서는 최근에 발표된 인크레틴 계열 약제(DPP-4 억제제 및 GLP-1 receptor agonist [GLP-1RA])의 심혈관질환 안전성에 대한 대규모 임상연구 결과를 정리하고, 각각의 약제별로 연구결과에서 보인 유사점과 차이점 및 각 약제의 심혈관계에 대한 간접적 그리고 직접적인 작용기전, 그리고 추후 당뇨병약제의 심혈관질환 안전성에 대한 연구를 할 때 고려할 점에 대해 정리해보고자 한다.

DPP-4 억제제

1. DPP-4 억제제의 혈당 조절에 대한 작용기전

DPP-4 억제제는 인크레틴 약물의 일종으로 최근에 개발된 제2형 당뇨병 치료 약제이다. 인크레틴이란, 췌장 베타세포의 인슐린 분비를 촉진하는 작용이 있는 장에서 나오는 호르몬을 말하며, 소장의 K세포에서 나오는 gastric inhibitory peptide (GIP)와 소장의 L-세포에서 나오는 GLP-1이 있다.11 인크레틴은 혈액으로 분비되면 DPP-4라는 효소에 의해 분해되어 비활성화되는데, 이를 억제하는 DPP-4 억제제는 혈액에 있는 인크레틴 호르몬의 분해를 억제하여 인슐린 분비 증가와 글루카곤 분비 억제 작용을 통해 혈당을 낮춘다. 그 외에 혈압을 감소시키고, 식후 중성지방을 감소시키는 효과가 있다.
현재 국내 사용이 가능한 DPP-4 억제제에는 sitagliprin, vildagliptin, saxagliptin, linagliptin, gemigliptin, alogliptin, teneligliptin, anagliptin, evogliptin이 있으며, 일본에서는 omarigliptin이 사용 중이고, dutogliptin은 현재 3상 임상 중이다. 약제별로 화학적 구조나 DPP-4 효소 억제 정도, 반감기 및 대사에 차이는 있으나 당화혈색소는 0.5%–0.6% 정도 감소시키며, 단독요법시 저혈당이나 체중 증가와 같은 부작용이 없고, 간질환이나 신질환 환자에서도 사용이 가능한 약제이다.

2. DPP-4 억제제의 심혈관질환에 대한 대규모 임상연구 결과

DPP-4 억제제 중에서 현재 saxagliptin (SAVOR-TIMI 53), alogliptin (EXAMINE), sitagliptin (TECOS)의 심혈관질환에 대한 대규모 임상연구 결과가 보고되었고(Table 1), linagliptin 연구 결과는 조만간 보고될 예정이다.

SAVOR-TIMI 53 연구는 심혈관질환의 병력이 있는 당뇨병 환자를 대상으로 saxagliptin의 심혈관질환 안전성을 알아보고자 진행된 연구이다. 일차 목표는 위약 대비 saxagliptin의 major adverse cardiovascular event (MACE) 발생에 대한 비열등성(안전성) 및 우월성(효과)를 알아보고자 하였다. 그 결과, MACE 발생에 있어서는 saxagliptin 투여 시 위약 대비 심혈관질환에 대한 안전성은 확인하였고, 우월성은 보이지 않았으며, 예상 밖으로 saxagliptin 투여군에서 심부전으로 인한 입원율이 유의하게 증가하였다.4
EXAMINE 연구는 최근 급성관상동맥증후군을 앓은 제2형 당뇨병 환자를 대상으로 위약대비 alogpitin의 심혈관질환 안전성을 평가하였다. 그 결과 위약대비 aloglitpin은 심혈관질환을 증가시키지 않은 안전성이 확인되었고, 이는 SAVOR 연구결과와 같았다. SAVOR 연구와 다른 점은 대상환자들이 급성관상동맥증후군을 앓은지 수주 이내의 급성기 환자를 대상으로 하여서 대상 환자 수도 적고, 추적기간도 짧았으며, 심부전에 대한 입원율이 증가하는 경향은 보였으나, 통계적으로 유의한 차이는 없었다.5
TECOS 연구는 심혈관질환이 있었던 당뇨병 환자에서 위약대비 sitagliptin의 심혈관질환의 안전성을 조사하였고, 일차 목표인 MACE에 있어서 역시 다른 연구와 마찬가지로 위약대비 sitagliptin의 비열등성을 입증함으로써 심혈관질환에 대한 안전성을 확인하였다. 앞의 두 연구와 다른 점은 MACE로 비치명적 심근경색, 비치명적 뇌경색, 심혈관질환의 사망률이외에 불안정성 협심증에 대한 입원율 지표가 포함된 점이며, 심부전에 대한 입원율도 sitagliptin 투여군에서 증가하지 않았다.6
세 가지 연구 이외에도 DPP-4 억제제 중에서 sitagliptin, saxagliptin, linagliptin, vildagliptin, alogliptin의 phase II–III 연구 결과들을 모아서 심혈관질환의 안전성을 알아보고자 pooled analysis 연구를 한 결과,12 심혈관질환에 대해 유의하게 감소시키는 효과가 보고되었다. 그러나 이러한 연구들은 대사지표를 보고자 한 연구이며 심혈관질환의 발생을 연구지표로 보고자 한 것이 아니기 때문에, 심혈관질환의 지표가 다소 정확하지 않다는 한계점이 있다. 또한 심혈관질환이 없는 비교적 건강한 당뇨병 환자들이 포함되었고 기간이 짧았기 때문에, 심혈관질환을 앓았던 사람보다는 그렇지 않은 사람에서 오히려 DPP-4 억제제가 심혈관질환의 발생을 줄일 수 있다고 볼 수도 있을지에 대한 추가적인 연구가 필요하다.
DPP-4 억제제와 심부전 입원율에 있어서 SAVOR 연구에서 증가된 결과에 대해서는 여전히 해석에 어려움이 있으며, 다른 약제와 달리 이 약제만의 특이점으로 볼지, EXAMINE, TECOS까지 같이 분석한 결과에서는 심부전의 입원율이 증가하지 않았기 때문에 class effect로 안전하다고 할지에 대한 논란의 여지는 여전히 남아 있다. 하지만 SAVOR 연구 자료에서 심부전에 대한 추가분석을 한 결과를 보면,13 심부전 입원율은 1) 이전에 심부전이 있거나, 2) 연구 시작 시점의 N-terminal pro B-type natriuretic peptide (NT-proBNP)가 높은 사람, 그리고 3) estimated glomerular filtration rate (eGFR)이 60 mL/min 이하인 경우에서 증가했기 때문에, 심부전이 있는 환자나 신기능이 저하된 환자에서는 심부전에 대해 주의하라는 FDA의 경고가 추가되어 있다.

3. DPP-4 억제제의 심혈관계 및 동맥경화증에 대한 작용기전

DPP-4 억제제 투여 후 심혈관계 위험인자인 혈당, 혈압, 이상지질혈증의 개선을 통한 간접적인 효과가 있다. 또한 DPP-4 효소는 transmembrane peptidase로 soluble form으로 존재하며 몇몇 사이토카인과 펩티드의 N-말단 부위에서 position2의 proline/alanine 잔기인 dipeptides를 자르는 역할을 한다. 따라서 DPP-4 억제제 투여 후 혈액에 증가된 GLP-1의 심혈관계에 대한 직접적인 작용으로 GLP-1이 심방이나, 혈관내피세포 및 대식세포에 있는 GLP-1 수용체에 결합하여 심장의 수축력 증가, 혈관확장작용 및 심근 허혈 조직의 회복개선, 항염증작용에 기여하며, 이는 DPP-4 효소의 기질 중에서 Table 2와 같이 peptide YY, neuropeptide Y (NPY), stromal cell-derived factor 1 (SDF-1), monocyte chemoattractant protein-1 (MCP-1) 등이 증가되어 심장의 수축력 증가, 혈관 확장 및 항염증에 대한 직접적인 작용을 하는 것으로 추정된다(Table 2).1415

GLP-1 수용체작용제

1. GLP-1 수용체작용제의 혈당 및 대사조절에 관한 작용기전

우리 몸의 GLP-1 은 DPP-4 효소에 의해 빠르게 분해되기 때문에 당뇨병 치료제로 사용하기 위하여 DPP-4에 의해 잘 분해되지 않고 반감기가 긴 GLP-1 수용체작용제가 개발되었다. 이 약제는 GLP-1의 수용체에 작용하여 DPP-4 억제제보다 훨씬 높은 농도의 GLP-1 수용체 자극을 통해 인슐린 분비 증가와 글루카곤 분비 억제 작용을 통해 혈당 감소효과가 있다. 또한 동물연구를 통해 GLP-1 수용체작용제는 췌장의 베타세포의 증식을 촉진하고 세포 자연사를 억제하며 췌관 세포에서 췌도 세포로의 분화를 촉진하는 작용이 보고되었다.16 그 외에 식욕 억제 효과, 그리고 위장관 운동 저하를 통해 음식 배출 지연 작용 등으로 혈당을 낮추고 체중을 감소시키는 작용을 한다. 혈당강하효과는 약 1%–1.5%의 당화혈색소를 감소시키며, 오심, 구토, 설사와 같은 소화기계 부작용이 흔하다.
이 약제는 크게 2가지 계열로 나뉘는데 한가지는 Gila monster라는 도마뱀의 독에서 추출한 exendin-4를 기반으로 한 약제로 사람의 GLP-1의 아미노산과 53% 정도 동일하며 exenatide, lixisenatide가 여기에 속한다. 다른 한가지는 사람의 GLP-1과 아미노산이 97% 가량 동일하며 사람의 GLP-1을 기반으로 한 약제로 liraglutide, semaglutide, dulaglutide, albiglutide가 여기에 속한다. 또한 GLP-1 수용체작용제를 작용시간에 따라서 속효성과 지속성으로 분류하는데 속효성 GLP-1 수용체작용제는 위장관 배출을 억제하여 식후 고혈당을 개선시키는 효과가 있으며 exenatide, lixisenatide가 여기에 속한다. 지속성은 인슐린 분비 증가 및 글루카곤 분비 억제 작용을 통해 공복 혈당을 개선시키는 효과가 우수하며 liraglutide, semaglutide, dulaglutide, albiglutide가 속한다. 또한, GLP-1 수용체작용제는 혈당 감소 이외에도 체중 감소, 저밀도지단백 콜레스테롤 (low-density lipoprotein cholesterol, 이하 LDL-C), 중성지방 감소와 같은 이상지질혈증 개선, 혈압 감소 효과가 있다(Table 3).

2. GLP-1 수용체작용제의 심혈관질환에 대한 대규모 임상연구 결과

GLP-1 수용체작용제 중에서 현재 lixisenitide (ELIXA), liraglutide (LEADER), semaglutide (SUSTAIN-6), exenatide 1 주제형 (EXSCEL)로 4개의 심혈관질환에 대한 대규모 임상연구 결과가 보고되었고(Table 4), dulaglutide와 albiglutide 연구 결과는 조만간 보고될 예정이다.

LEADER 연구는 이전에 심혈관질환을 앓았거나 심혈관질환의 위험도가 높은 제2형 당뇨병 환자에서 liraglutide 투여는 심혈관사망률, 비치명적인 심근경색, 비치명적인 뇌졸중인 일차 목표를 위약대비 13% 유의하게 감소시키는 효과를 입증하였으며, 특히 이 효과는 심혈관질환의 사망률을 유의하게 감소시켜 나타났다.8 그러나 liraglutide 치료가 당화혈색소 및 수축기혈압을 유의하게 감소시킨 것에 비해 혈압이나 혈당의 감소 정도가 위약대비 크지 않다는 점에서 심혈관질환의 위험을 줄일 만큼 크게 기여하지 않았을 것으로 예상되며, 고혈압, 고혈당과 같은 전통적인 심혈관질환의 위험인자 이외에 다른 기전이 있을 것으로 추정된다.
SUSTAIN-6 연구는 심혈관질환의 위험이 높은 당뇨병 환자를 대상으로 1주 한번 투여하는 semaglutide가 위약대비 심혈관사망률, 비치명적인 심근경색, 비치명적인 뇌졸중인 일차 목표를 위약대비 26% 유의하게 감소시켰다. LEADER 연구에서는 비치명적 심근경색이나 비치명적 뇌졸중의 발생보다는 심혈관질환의 사망률을 유의하게 감소시켰던 반면에, SUSTAIN-6 연구에서는 semaglutide가 비치명적인 심근경색과 뇌졸중을 유의하게 감소시켜 다소 다른 결과를 보였다.9
EXSCEL 연구는 심혈관질환의 위험이 높은 제2형 당뇨병 환자를 대상으로 1주일에 한 번 투여하는 exenatide 투여가 위약대비 심혈관질환의 발생을 낮추는데 중립적인 효과를 보였다.10 연구 초기에 사용된 약제의 주사 디바이스가 복잡해서 조기 중단이 많았던 이유로 연구가 조기 종료함에 따라 우월성을 보이지 못했을 것으로 생각된다.
최근에 있었던 GLP-1RA의 심혈관질환 임상연구 결과를 보면 약제마다 서로 상이한 결과를 보였는데 그 이유는 다음과 같이 설명할 수 있다. 첫째로, 대상 환자가 다르다. ELIXA 연구는 급성관상동맥증후군이 연구등록 얼마 전에 있었던 환자를 대상으로 했고, LEADER나 SUSTAIN-6 연구에서는 기존에 심혈관질환을 앓았거나, 심혈관질환의 위험인자를 많이 가지고 있는 사람을 대상으로 하였는데, 대상환자의 나이가 더 많고 당뇨병 유병기간이 더 길었으며 당화혈색소와 body mass index (BMI) 수치가 더 높았다. 따라서 ELIXA 연구에서는 대부분의 환자들이 스타틴, 베타차단제, 항혈소판제를 복용하고 있었다. LEADER 연구의 서브그룹 분석에 의하면 연구 시작 시 BMI가 높았던 군에서 심혈관질환의 발생이 더 낮았지만, SUSTAIN-6에서는 그렇지 않았다. 또한 신장기능이 낮은 사람에서 심혈관질환 예방 효과가 LEADER 연구에서는 더 컸지만, ELIXA 연구에서는 그렇지 않았다. 둘째로 심혈관질환의 위험인자에 미치는 효과에 차이가 있었다. ELIXA나 LEADER 연구에서는 연구 시작 시 HbA1C가 더 낮았고, 위약대비 당화혈색소의 감소 정도도 적었지만, SUSTAIN-6 연구에서는 당화혈색소의 감소 정도가 더 컸다. 수축기혈압의 차이도 또한 ELIXA나 LEADER 연구에서는 위약대비 적었지만, SUSTAIN-6 연구에서는 더 컸다. 체중 감소 효과는 LEADER에서 ELIXA 연구보다 더 컸고 SUSTAIN-6는 비슷했다. 지방대사에 대한 자료가 ELIXA, LEADER 연구에서는 보고되지 않았지만 SUSTAIN-6 연구에서는 중성지방 및 high-density lipoprotein cholesterol에 유의한 차이가 있었다. 게다가 liraglutide나 semaglutide는 lixisenatide와 달리 맥박 수를 증가시켰는데, 이는 오히려 심혈관 위험에 나쁘게 작용했을지도 모른다. 셋째로는 약동학적 차이가 있다. Lixisenatide는 속효성이고 liraglutide나 semaglutide는 지속성이다.

3. GLP-1 수용체작용제의 심혈관질환계에 대한 작용기전

심혈관계 위험인자인 혈당 감소, 혈압 감소 및 LDL-C 감소와 중성지방 감소와 같이 이상지질혈증의 개선을 통한 간접적인 효과가 있다. GLP-1 수용체작용제가 GLP-1 수용체와 결합하면 전형적인 GLP-1 수용체 경로를 통해 심근수축력 증가, 관상동맥 혈류증가, 심근세포의 포도당 섭취 증가를 일으키며, GLP-1 수용체와 독립적으로 DPP-4에 의해 불활성화된 GLP-1(9–36)은 허혈에 의한 심근세포 손상을 감소시키고 혈관확장작용에 기여함이 보고 되었다.17 마지막으로 GLP-1 수용체를 통해 대식세포에서 염증을 줄이고 혈관내피세포에 단핵구의 침투를 줄이고 내피세포의 세포자연사를 감소시켜 항동맥경화작용이 있음이 보고되었다.18

결론

최근에 개발된 DPP-4 억제제와 GLP-1 수용체작용제의 심혈관질환 안전성에 대한 대규모 임상연구 결과, DPP-4 억제제는 심혈관질환을 올리지도 낮추지도 않는 안전함을 보여 주였고, GLP-1 수용체작용제 역시 안전성을 입증하였고, 추가로 liraglutide, semaglutide는 심혈관질환의 발생을 유의하게 감소시킨다는 긍정적인 효과가 확인되었다. 인크레틴-기반 약물 치료의 심혈관질환 안전성을 넘어서 유의하게 감소시키는 효과가 보이는 기전으로는 고혈당 이외에 고혈압 및 이상지질혈증을 개선시키고, 체중을 줄이는 대사적인 측면 이외에도 GLP-1에 의한 심장, 혈관, 면역세포에 관여하는 직접적인 효과가 같이 기여했을 것으로 생각된다. 같은 계열이라도 약제 간에 차별점이 관찰되어 이에 대한 자세한 기전은 추가적인 연구가 필요할 것으로 생각된다.
