Kim, Cho, Whang, and Chung: Treatment of supracondylar fractures of the humerus by percutaneouspinning in children

Abstract

The supracondylar fracture of the humerus is the most common fracture about the elbow in children and often associated with variable complications.
During the period from 1983 to 1989, 77 children with supracondylar fractures of the humerus which could be followed for more than 6 months were treated by closed reduction and percutaneous pinning.
The result of clinical survey of 77 children was presented. The resuit was as follows:
1. The average age of children was 6.8 years, male and female ratio was 4:1.
2. Seventy-fou. cases (96%) were the extension type and 3 cases(4%) we.e the flexion type. In extension type, 43 cases(58%) were Gartland type II, 23 cases (31%) were Gartland type III-A and 8 cases(11%) were Gartland type III-B.
3. In the pattern of percutaneous pinning, the crossed pinning by 2 Kirschner-wires was 60 cases (78%), the paralleled pinning by 2 Kirschner-wires was 5 cases(7%), the crossed pinning by 3 Kirschner-wires was 11 cases(14%)and paralleled pinning by 3 Kirschner-wires was 1 case(1%).
4. Complications included transient ulnar nerve injury 9 cases, pin tract infection 1 case, cubitus varus deformity 2 cases, migration of pin 2 cases and redisplacement 2.
5. In functional results according to Flynns method, the satisfactory results were 73 cases (95%), and the unsatisfactort results were 4 cases(5%).
