Kim, Lee, and Kim: Cases of bipolar endoprosthetic replacement of unstable intertrocha nteric femoral fracture in the elderly, osteoporotic patients

Abstract

As we well aware, the intertrochanteric fractures of the femur usually may take place frequently in the elderly group. When the unstable comminuted intertrochanteric fracture of the femur is internally fixed, 1) the initial reduction of the fracture site is possible, but the malunion of the varus deformity on the fracture site may occur on early weight bearing. 2) the delayed weight-bearing until the bony union of the fracture site may Induce the delayed rehabilitation. 3) the internal fixation is unstable to the weak medial cortex of proximal femur 4) the complication and death rate after long immobiliza-tion may increase in the elderly patients. In our department of orthopaedic surgery for the sake of promoting early full weight bearing and rapid rehabilitation, we treated five elderly patients with Singhs Index Grade III and unstable comminuted intertrochanteric femoral fracture, by means of bipolar endoprosthesis rather than sliding-compression scerw-plate or Ender nails. As results, 3 elderly patients were capable to be early recovered and return to early welght bearing status and rapid rehabilitation.
