Capillary hemangioma of the cauda equina is extremely rare. We present the MR imaging and histologic findings of a case of this disease. The tumor was well demarcated, 0.9×1.3 cm in diameter and 1.2 cm in length. On the MR images, the tumor showed isointensity relative to the spinal cord on the T1-weighted images, hyperintensity on the T2-weighted images, and strong homogeneous enhancement on the contrast-enhanced T1-weighted images. Capillary hemangioma should be included in the differential diagnosis of a spinal tumor of the cauda equina.
