Lee and Ha: Ipsilateral Fracture of the Femoral Neck and Shaft

Abstract

Fractures of the femoral shaft associated with fractures of the femoral neck are uncommon injuries with the difficulties in diagnosis and treatment. The femoral neck component of this injury is commonly missed initially. It should be carefully searched for, especially in those patients in whom physical examination can not be done adequately. This combination of fractures occures predominently in young people as a result of severe trauma. These patients frequently have associated injuries to the head, abdomen, and other musculoskeletal system. We reviewed the cases of 9 patients who were treated at Orthopaedic Department, Chosun University Hospital from Jan. 1985 to Dec. 1990. Although the following period is not long enough, the results were satisfactory. The results were as follows; 1. Careful clinical & X-ray evaluation of the joint above & below .a femoral shaft fracture will prevent such missed diagnosis. 2. The majority of our patient were involved in motor vehicle accidents; 6 were motorcyclist, 2 were bicyclist struck by automobiles. Their ages ranged from 22 to 49 years (average age 34 years). 3. In 9 cases of the femoral neck fractures, 4 which were unrecognized at initial examination in early phase. 4. Ipsilateral injuries to the knee occured in three patients, we obtained full range of motion in the involved knee postoperatively. 5. There were complications such as 1 case of delayed union of femoral shaft, 1 case of partial ankylosis of knee and 1 case of nonunion of femoral shaft. But, all cases of femoral shaft & neck fractures united. On the basis of experience with 9 cases, the recommended treatment consists of stalbe internal fixation even though femoral neck fracture was missed initially.
