The lumbar apophyseal fracture is an uncommon injury that occurs usually in children and adolescents. We described one case occurs in young adult with back and radiating pain. Disc herniations in adolescents are very rare and some of them are due to fracture of the posterior lumbar apophyseal ring. We report a case of simultaneous fractures of the third and fourth posterior lumbar vertebrae. To the best of our knoweldge, this case is unique a point of two lumbar level.
