Kwon, Kim, and Choi: Early Experience of Oganesyan Treatment

Abstract

It is still a very difficult process to restore the normal movement of joints of a patient who suffer from joint stiffness or flexion contracture, although many treatments have been contrived. The Oganesyan external fixator, which was invented by Mstislav V. Volkov and Oganes V. Oganesyan in 1971, generally employes the hinged-distraction apparatus to treat joint stiffness or flexion contracture and the reposition-fixation apparatus to treat fracture, nonunion, malunion, and limb lengthening. The Oganesyan treatment has not yet been widely introduced in the Western countries. Its merits and demerits need to be learned and the problems found during the therapeutic process need to be studied. We intend to report on the therapeutic process and result of the Oganesyan treatment and discuss its merits and demerits, based on the cases of 36 patients we applied the treatment from September 1991 to May 1992. The results were as fellows; 1. The ages of the patients ranged from 7 to 64 2. There were 11 cases of joint deformity and joint contracture, 21 cases of tibiofibular fracture and nonunion and 4 cases of the lengthening of tibia and fibula. 3. There were 11 cases of joint deformities and joint contractures, 7 cases were the foot and ankle joint deformities and joint contractures, 2 cases were the interphalngeal joint dislacations, one of the knee joint and one of the elbow joint. 4. For the treatment of the fresh tibia & fibula fractures, we could easily reduce the fracture by using the reposition-fixation apparatus of Oganesyan treatment. 5. The Oganesyan treatment showed many merits in treating joint contractures and fresh fractures. The operation procedure was relatively simple and caused little or no pain to the patient. It also was very convenient to the patient. 6. Even though we had a few cases of limb lengthening-4 cases, we found no sighificant merits of the Oganesyan treatment in comparison with the Ilizarov treatment.
