Kim, Whang, Choi, and Kim: Milch Osteotomy for the Correction of Cubitus Valgus and Cubitus Varus Deformities

Abstract

Cubitus valgus and cubitus varus deformities resulted mostly from malunion, nonunion, and physeal injury following the fracture of elbow in children. Although the function of the elbow is not greatly impaired, the patient or parents complained of the cosmetic problem and many techniques were introduced for the correction of the deformity. Milch osteotomy (opening wedge displacement osteotomy) for the correction of cubitus valgus and cubitus varus was carried out in 14 patients from Jan. 1986 to Sep. 1991. There were nine cubitus varus and five cubitus valgus deformities, and the results were as follows. 1. The most common cause of cubitus varus was malunion of supracondylar fracture of the humerus and that of cubitus valgus was nonunion of lateral condylar fracture of the humerus. 2. The results of Milch osteotomy were as follows; 8 cases were excellent, 2 cases good, and 4 cases poor. In pediatric group, six patients among seven were classified excellent and good result. In summary, we suggest Milch osteotomy might be one of the excellent technique for the correction of cubitus deformity, in which the coronal and rotational deformities were corrected simultaneously, and also the protruding condylar deformity was decreased.
