<p>Lumbar spondylolysis is a frequent cause of low back pain especially in adolescents involved in sporting activities. It is considered as a fatigue-type defect in the pars interarticularis or isthmus resulted from repetitive hyperextension and rotation on the spine; however, there is still confusion in regard to imaging findings of the location and direction of the defect. We report two unique cases of fifth lumbar spondylolysis in professional baseball hitter and adolescent pitcher; early-stage unilateral incomplete isthmus fracture and bilateral fracture in a different configuration, respectively. Computed tomography demonstrated vertical and more coronally oriented fracture lines compared with typical spondylolytic defect, and repetitive rotation in the same direction of pitching might cause a different type of fracture on the contralateral isthmus with the preexisting unilateral defect. Intriguingly, early-stage unilateral fracture in hitter was united after only 6 weeks of rest and rehabilitation and able to return to the game.</p>

<title>서 론</title>
<p>척추분리증은 유소년이나 청소년기 만성 요통의 흔한 원인으로 선천적 내지는 유전적 소인에 기인할 수도 있으나<sup><xref rid="ref1" ref-type="bibr">1</xref></sup>, 척추의 과도한 신전이나 회전 운동을 유발하는 스포츠 선수에서 척추 후궁 관절간부(pars interarticularis) 또는 협부의 피로 골절로 인한 경우가 많다<sup><xref rid="ref2" ref-type="bibr">2</xref></sup>.</p>
<p>하부 요추, 특히 제5요추(the fifth lumbar vertebra, L5)에 흔하고 양측성인 경우가 많으며, 주로 가관절 형태의 진행된 단계에서 발견된다<sup><xref rid="ref3" ref-type="bibr">3</xref></sup>. 그러나 척추분리증이 성인에서 급성으로 발생한 경우는 드물며<sup><xref rid="ref4" ref-type="bibr">4</xref></sup>, 척추분리증의 발병 기전이 신선 골절과 달리 피로 골절임을 감안하면 초기 발생단계 시 협부의 골절 시작 부위나 골절의 진행 방향, 또한 양측성 척추분리증의 경우 양측 피로 골절부의 형태적 차이와 관련한 국내 보고는 찾아보기 힘들다.</p>
<p>저자들은 각각 프로야구 선수에서 발생한 초기 편측성 척추분리증과, 청소년 야구 선수에서 발생한 좌우 협부 피로 골절의 형태가 상이한 특이한 척추분리증을 경험하였기에 문헌 고찰과 함께 보고하고자 한다.</p>
</sec>
<sec sec-type="cases">
<title>증 례</title>
<p>본 증례 보고는 은성의료재단의 임상연구심의윤리위원회의 승인(No. GGAH 2021-03, 2021-05) 후 진행하였다.</p>
<sec>
<title>1. 증례 1</title>
<p>30세 남자 프로야구 외야수(신장 186 cm, 체중 101 kg, 우투우타)가 내원 1개월 전부터 발생한 허리 통증을 주소로 내원하였다. 하지 방사통이나 야간통, 안정 시의 동통은 없었으나 타격 시 체간 회전이나 트레이닝 동작 중 스쿼트 자세에서 요통이 발생하였고, 최근 1주일 동안 악화되어 시즌 시작 엔트리에서 제외되었다. 수술력이나 외상력은 없었고, 하지 근력이나 감각을 포함한 신경학적 검사와 혈액 순환은 정상이었다. 좌측 하부 요추 기립근 주위로 경미한 압통이 있었으며 하지 직거상 검사상의 제한은 없었다. 단순 방사선검사에서 특이 소견은 없었고, 타 원에서 촬영한 자기공명영상에서 추간판의 탈출은 없었으나 지방 억제 시상면 영상에서 정상적인 건측(<xref rid="F1" ref-type="fig">Fig. 1A</xref>)과는 달리 L5 좌측 척추 경과 상돌기 부위에 고강도 신호의 골수 부종이 저강도 신호의 골절 선 주위로 관찰되었다(<xref rid="F1" ref-type="fig">Fig. 1B</xref>). 전산화단층촬영 수평면 상 L5 좌측 협부에 인접하여 골절선이 관찰되었는데(<xref rid="F2" ref-type="fig">Fig. 2A</xref>), 이는 통상적인 척추분리증의 협부 결손부보다 전방에 위치하였고 시상면 상 건측에 비해 척추 경 후하방 피질골의 부분 단절 양상이었다(<xref rid="F2" ref-type="fig">Fig. 2B and C</xref>). 피로 골절로 인한 편측성 척추분리증으로 진단하고 훈련이나 트레이닝을 제한하였으며 보조기 고정은 별도로 시행하지 않았다. 진단 3주 후 척추 회전 시 통증이 없어 가벼운 티배팅을 시작하였고 중심 안정성 운동을 병행하였다. 5주에 수비 기술 훈련과 타격 훈련을 시작하였는데 장거리 투구 시에도 통증이 없었으며, 복귀 전 시행한 전산화단층촬영 수평면(<xref rid="F3" ref-type="fig">Fig. 3A</xref>) 및 시상면 상(<xref rid="F3" ref-type="fig">Fig. 3B</xref>)에서 피로 골절부는 유합되어 6주에 정상적인 경기 참여가 가능하였다.</p>

<sec>
<title>2. 증례 2</title>
<p>14세 중학교 2학년 남자 투수로(신장 166 cm, 체중 72 kg, 우투우타) 7개월 전부터 투구 시 간헐적으로 발생한 요통 및 좌측 둔부통, 좌 슬근의 경직감을 주소로 내원하였다. 요통은 내원 1개월 전 동계 훈련 시 악화되어 훈련에 불참하고 사설 운동센터에서 재활 중이었다. 진찰 시 좌측 후방 장골능 부위로 압통이 있었으며 하지 직거상 검사상 좌측이 80&#176;로 제한되어 있었다. 신경학적 이상은 관찰되지 않았고, 좌측 편각 기립 검사 시 좌측 편향을 보였으며 자세 유지에 어려움이 있었다. 사면 단순 방사선 검사에서 우측은 L5 협부에 전형적인 Scotty dog 결손이 관찰되었으나(<xref rid="F4" ref-type="fig">Fig. 4A</xref>), 좌측은 협부 결손 없이 하관절 돌기로 골절선이 연장되어 있었다(<xref rid="F4" ref-type="fig">Fig. 4B</xref>). 자기공명영상에서 L5 우측 협부는 고강도 신호의 결손을 보였으나(<xref rid="F5" ref-type="fig">Fig. 5A</xref>), 좌측은 척추경 부위로 고강도 신호의 골수 부종과 저강도 신호의 골절선이 관찰되었다(<xref rid="F5" ref-type="fig">Fig. 5B</xref>). 골절 양상은 전산화단층촬영 수평면 상 L5 우측 협부는 후측방 방향, 좌측은 횡측 방향이었으며(<xref rid="F6" ref-type="fig">Fig. 6A</xref>), 시상면 상에서도 전형적인 사선형 배복 방향(dorsoventral)의 우측 결손(<xref rid="F6" ref-type="fig">Fig. 6B</xref>)과는 달리 좌측은 수직 방향으로 상부 후관절까지 연장되어 있었다(<xref rid="F6" ref-type="fig">Fig. 6C</xref>). 골절선이 예리하지 않고 골절부 주위 경화상과 골 흡수 소견이 혼재되어 급성 골절로는 판단되지 않아 수술이나 보조기 고정은 시행하지 않았으며, 투구 휴식과 함께 슬근 경직을 완화시키고 요추부 유연성과 체간의 중심 안정성을 강화하는 방향으로 재활을 시행하였다. 요통 및 좌측 둔부통은 치료 시작 4주에 호전을 보였으며, 8주에 슬근 경직 증상과 편각 기립 검사의 호전을 보여 기간 투구 프로그램을 진행하였고 3개월에 점진적 투구 복귀가 가능하였다. 추시 영상 촬영은 원하지 않았으며 6개월 경과 후에도 증상 재발은 없어 외래 추시 관찰 중에 있다.</p>

<sec sec-type="discussion">
<title>고 찰</title>
<p>척추분리증은 청소년기 운동 선수 요통의 중요한 원인으로, 급성 또는 만성 구분없이 역도, 체조, 육상 선수 등에서 40% 가까이 관찰되는 질환이다<sup><xref rid="ref4" ref-type="bibr">4</xref>,<xref rid="ref5" ref-type="bibr">5</xref></sup>. 그러나 야구 선수에 대한 보고는 드물며, 특히 국내 프로야구 선수에 대한 자료나 보고가 없어 진료 시 참고하기가 어렵다. 척추분리증은 진행 단계에 따라 초기, 진행기, 종말기로 구분되는데<sup><xref rid="ref4" ref-type="bibr">4</xref></sup>, 성인 척추분리증의 대부분은 협부 골절의 불유합성 가관절 형태인 종말기에서 발견된다. 그러나 Tezuka 등<sup><xref rid="ref4" ref-type="bibr">4</xref></sup>은 운동 선수에서 급성이나 아급성 피로 골절 형태의 척추분리증 11예를 보고하였는데, 프로축구(3예), 육상(4예), 체조(1예) 선수를 제외하고 프로야구 선수가 3예(21&#8211;25세)로, 과도한 투구 연습을 했거나 투구 폼을 변경한 투수와 타격 훈련 중인 포수에서 발생하였고, 골절의 형태도 본 증례 1과 유사한 편측성 초기 협부 피로 골절뿐만 아니라, 기존의 편측 척추분리증을 가진 상태에서 반대측 협부에 발생한 골절, 하부 요추의 척추분리증에 추가로 발생된 상부 요추의 협부 골절 등 여러 형태가 있을 수 있다고 하였다.</p>
<p>운동 선수에서 발생하는 협부 피로 골절의 원인은 명확하지는 않으나 협부의 해부학적 취약성과 척추의 굴신 및 회전 시에 협부에 가해지는 응력 집중과 관련이 있다<sup><xref rid="ref2" ref-type="bibr">2</xref>,<xref rid="ref6" ref-type="bibr">6</xref></sup>. 특히 장타력을 가진 증례 1 선수의 비우세측인 좌측 협부에 발생한 점으로 보아, 타격 동작 시 과도한 체간 회전으로 유발되는 비우세측의 내복사근 파열과 유사한 기전으로 기계적 취약부인 협부 하방에 부하가 가해지는 것으로 생각할 수 있다<sup><xref rid="ref7" ref-type="bibr">7</xref></sup>. 또한 Sairyo 등<sup><xref rid="ref6" ref-type="bibr">6</xref></sup>의 유한 요소 모델 연구에 의하면 편측 척추분리증이 있는 경우 정상 척추에 비해 반대측 협부나 척추경에 가해지는 부하가 12.6배로 증가한다고 하였고, Yamashita 등<sup><xref rid="ref8" ref-type="bibr">8</xref></sup>도 양측의 진행 단계가 상이한 청소년 운동 선수 척추분리증 5예(9&#8211;20세) 보고에서 종말기 협부 결손측보다 초기 결손측이 증상을 유발하는 주된 원인이라고 하였다. 본 증례 2의 경우 요통과 더불어 좌측 둔부통과 좌슬근 경직감, 좌측 편각 기립 검사상의 이상을 보인 것은 전위가 다소 진행된 만성적 우측 협부 결손에 비해 골절선의 형태로 보아 급성은 아니지만 우측보다 늦게 발생한 비우세측 좌측 협부 골절로 인해 유발된 것으로 판단된다.</p>
<p>초기 단계의 척추분리증은 진단이 쉽지 않은데, 흔히 요통을 보이는 성인 운동 선수의 경우 추간판 질환의 여부만을 확인하는 경우가 많으나 증례 1과 같이 자기공명영상에서 척추경 주변의 고강도 신호 변화는 협부 피로 골절을 의심할 수 있는 중요 소견이다<sup><xref rid="ref8" ref-type="bibr">8</xref></sup>. 컴퓨터단층촬영은 골절부의 형태뿐 아니라 질환의 단계를 판단할 수 있는 유용한 검사이며, 특히 증례 1의 경우 부분 골절부가 시상면과 상에서 기존의 협부보다 다소 전하방에 위치하여 척추경의 피로 골절로도 오인할 수 있다. 두 증례 모두 주된 증상을 유발하는 좌측 협부의 골절면이 척추 시상면을 기준으로 상하 방향이고, 특히 증례 2는 골절면이 척추 관상면 방향인 횡측으로 발생한 점은 이 증례들이 기존의 척추분리증에 비해 비전형적이며 특이하다고 할 수 있는데, 생역학적 연구상 반복적인 투구나 타격 동작 시 하체로부터 전달되는 에너지가 요추의 회전 및 신전으로 인해 전단력이 더해져 관상면 방향의 골절을 더 많이 유발할 수 있다는 보고가 있다<sup><xref rid="ref9" ref-type="bibr">9</xref></sup>.</p>
<p>척추분리증은 보존 치료가 우선적이며 초기와 진행기 협부 골절의 일반적 유합 시기가 각각 3개월, 6개월 정도임을 감안하면<sup><xref rid="ref4" ref-type="bibr">4</xref>,<xref rid="ref10" ref-type="bibr">10</xref></sup>, 증례 1은 비교적 빠른 내원 6주에 유합을 얻을 수 있었는데, 이는 질환의 조기 진단이 중요한 역할을 한 것으로 생각된다. 비록 증례 2의 추시 영상 부재로 유합 여부를 확인할 수 없으나 종말기에 가까운 골절이므로 유합은 기대하기 어려웠을 것으로 생각된다. 또한 장기적인 추시를 통해 불유합이 지속되어 척추 전방전위증으로 진행하거나 이로 인한 신경학적 이상이나 보존적으로 조절되지 않는 요통이 1년 이상 지속될 경우 척추유합술의 적응이 될 수 있을 것으로 생각한다<sup><xref rid="ref5" ref-type="bibr">5</xref>,<xref rid="ref10" ref-type="bibr">10</xref></sup>. 저자들은 이번 증례를 통해 청소년 야구 선수 및 성인 프로야구 선수에서 발생하는 척추분리증의 기전과 형태적 다양성을 보고함으로써 적절한 진단과 치료를 통한 조기 복귀에 참고 자료가 될 수 있을 것으로 생각된다.</p>
