<abstract xml:lang="ko">
<p>충격파를 만드는 방법은 전기 수력(electrohydraulic), 전자기(electromagnetic), 압전기(piezoelectric) 등의 원리로 만들어진다. 체외충격파 치료는 기계적인 힘으로 목표 조직에 에너지를 집중시키는 효과와 조직 치유를 촉진하는 생물학적 효과가 있는 것으로 알려져 있다. 체외충격파 치료는 오랜 기간 시행되어 왔으며, 체외충격파 치료를 시행하는 분야는 석회성 또는 비석회성 건염, 주관절의 외상과염 또는 내상과염, 슬개건염, 아킬레스 건병증(achilles tendinosis), 근위부 족저근막염, 근막통증 증후군 등이다. 또한 골절의 지연 유합이나 불유합, 그리고 대퇴골 두의 무혈성 괴사, 당뇨병에 의한 피부 괴사 등에 시도되고 있다. 근골격계 질환에 대한 체외충격파 치료의 결과가 좋고 합병증도 경미하다는 보고가 많아 근골격계 질환에 체외충격파 치료의 사용이 증가하고 있다. 따라서 저자는 근골격계 질환에 대한 체외충격파 치료에 대하여 논문 고찰과 함께 정리해보고자 한다.</p>

<p>The sources of shockwave generation include electrohydraulic, electromagnetic and piezoelectric principles, and extracorporeal shock wave therapy (ESWT) appears to have mechanical and biological effects on tissue healing. The application of ESWT to musculoskeletal disorders has been around for more than a decade and is used primarily in the treatment of calcific or non-calcific tendinitis of the shoulder, lateral and medial epicondylitis of the elbow, patellar tendinopathy, Achilles tendinitis or proximal plantar fasciitis of the heel, myofascial pain syndrome, etc. ESWT is also used in the treatment of delayed union or non-union of long bone fractures, avascular necrosis of the femoral head, and chronic diabetic ulcers. The vast majority of papers have reported positive and beneficial effects with few complications. The clinical application of ESWT has increased steadily. This article reviews the current status of ESWT in musculoskeletal disorders.</p>

<title>서론</title>
  <p>충격파는 물과 같은 매질을 통해 고전압 폭발이나 증발로 발생하는 고 에너지 음파다.<xref ref-type="bibr" rid="B1">1)</xref> 충격파를 만드는 법은 전기수력, 전자기, 압전기 등의 원리로 만들어지며, 방사형과 초점형으로 나뉜다.<xref ref-type="bibr" rid="B2">2</xref><xref ref-type="bibr" rid="B3">3)</xref> 방사형은 퍼져나가기 때문에 충격파의 에너지를 조직의 한 곳에 집중할 수 없으나 조직에 전반적으로 체외충격파를 전달하는 효과가 있기 때문에 근육 질환에 많이 사용된다. 초점형은 조직의 한 부위에 충격파 에너지가 집중되는 효과가 있지만 이로 인하여 통증이 상대적으로 더 심할 수 있다. 따라서 최근에는 충격파 에너지가 한 점에 해당하는 부위에 집중되지 않고 동전 크기만한 부위에 충격파 에너지가 골고루 영향을 줄 수 있는, 따라서 통증을 경감할 수 있는 스마트 초점형이 개발되었다. 체외충격파 치료는 기계적인 힘으로 목표 조직에 에너지를 집중시키는 효과와 조직 치유를 촉진하는 생물학적 효과가 있는 것으로 알려져 있다.<xref ref-type="bibr" rid="B4">4</xref><xref ref-type="bibr" rid="B5">5)</xref> 체외충격파 치료를 시행할 수 있는 분야는 석회성 또는 비석회성 건염, 주관절의 외상과염 또는 내상과염, 슬개건염, 아킬레스 건병증, 근위부 족저근막염 등이다. 또한 골절의 지연 유합이나 불유합, 그리고 대퇴골 두의 무혈성 괴사, 당뇨병에 의한 피부 괴사 등이다. 이러한 질환에 대한 체외충격파 치료의 효과가 좋아 비수술적 치료 방법으로 유용하다고 많이 보고되고 있다.<xref ref-type="bibr" rid="B1">1</xref><xref ref-type="bibr" rid="B2">2</xref><xref ref-type="bibr" rid="B3">3</xref><xref ref-type="bibr" rid="B4">4</xref><xref ref-type="bibr" rid="B5">5</xref><xref ref-type="bibr" rid="B6">6</xref><xref ref-type="bibr" rid="B7">7</xref><xref ref-type="bibr" rid="B8">8</xref><xref ref-type="bibr" rid="B9">9</xref><xref ref-type="bibr" rid="B10">10</xref><xref ref-type="bibr" rid="B11">11</xref><xref ref-type="bibr" rid="B12">12</xref><xref ref-type="bibr" rid="B13">13</xref><xref ref-type="bibr" rid="B14">14</xref><xref ref-type="bibr" rid="B15">15</xref><xref ref-type="bibr" rid="B16">16</xref><xref ref-type="bibr" rid="B17">17</xref><xref ref-type="bibr" rid="B18">18)</xref></p>

<sec>
<title>체외충격파를 만드는 원리</title>
  <p>방사형 체외충격파는 공기압을 이용해 압력파(pressure wave)를 만드는 방식으로 발사체(projectile)에서 압축 공기를 가속화시킨 후 applicator에서 피부에 운동에너지를 전달한다(<xref ref-type="fig" rid="F1">Fig. 1</xref>).<xref ref-type="bibr" rid="B3">3</xref><xref ref-type="bibr" rid="B19">19)</xref> 퍼져나가기 때문에 충격파의 에너지를 조직의 한 곳에 집중할 수 없으나 조직에 전반적으로 체외충격파를 전달하는 효과가 있기 때문에 근육질환에 많이 사용된다.<xref ref-type="bibr" rid="B3">3)</xref> 초점형 체외충격파는 전기 에너지를 기계적 에너지로 전환시켜 충격파를 만든 뒤 반사판을 이용해 특정 부위에 충격파를 집중시키는 방식이다. 사람의 몸이 물과 비슷한 음향 임피던스(acoustic impedance)를 갖추고 있어 사람의 몸이 매질이 되어 충격파를 전달하는 원리이다. 조직의 한 부위에 충격파 에너지가 집중되는 효과가 있지만 이로 인하여 통증이 상대적으로 더 심할 수 있다. 초점형 충격파를 만드는 방법으로는 전기 수력(electrohydraulic), 전자기(electromagnetic), 압전기(piezoelectric) 등의 원리가 있다(<xref ref-type="fig" rid="F2">Fig. 2</xref>).<xref ref-type="bibr" rid="B2">2</xref><xref ref-type="bibr" rid="B19">19)</xref> 전기 수력의 원리로 만들어진 체외충격파 기계는 1세대로서 고압의 전기를 이용한 수중 폭발을 일으켜 발생한 충격파를 반사체를 이용하여 하나의 초점에 모이게 하는 장치이다. 전자기나 압전기 등의 원리로 만든 기계보다 상대적으로 더 큰 에너지를 내는 장점이 있다. 전자기 원리를 이용한 장치는 전류를 코일을 통과시키면서 강한 전기장이 형성되면서 충격파를 만들어 이를 렌즈를 이용하여 초점을 형성한다. 압전기를 이용한 장치는 전류 공급에 따라 부피가 수축과 팽창을 하는 압전 금속(piezoelectric crystals)이 주변의 수중에서 충격파를 발생시킨다. 형성된 충격파가 바로 초점을 형성(selffocus)할 수 있게 압전 금속이 기하학적으로 정렬되어 있다.<xref ref-type="bibr" rid="B2">2)</xref></p>
  <p>충격파는 단상성(uniphasic)으로 초음파의 이상성(biphasic)과 차이가 있다. 충격파의 최고 압력은 500 bar 정도이고 초음파의 최고 압력은 0.5 bar 정도로, 충격파가 1,000배 이상 더 큰 특징이 있다. 충격파의 효과는 두 가지로 나뉘는데, 하나는 기계적인 힘으로 목표 조직에 에너지를 집중시켜 치료 효과를 나타내는 것이고, 다른 하나는 공동화(cavitation) 현상으로 간접적인 기계적 힘 때문에 조직에 부정적인 효과 또는 손상을 주는 것이다.<xref ref-type="bibr" rid="B1">1)</xref></p>

<sec>
<title>체외충격파의 치료 기전과 동물실험</title>
  <p>체외충격파의 명확한 치료 기전은 아직 밝혀지지 않았다. 쇄석술에서는 결석을 분해하는 효과가 있는 반면, 근골격계 충격파는 조직을 분해하는 것이 아니고 간질 조직에 미세한 반응을 일으켜 치유를 촉진하는 것으로 생각된다.<xref ref-type="bibr" rid="B1">1)</xref></p>
<sec>
<title>1. 골절 치유</title>
  <p>뼈에 충격파를 가하면 미세골절을 만들어 혈종을 형성하고 조골세포(osteoblast)의 골형성을 유도하며 골절을 치유하는 것으로 생각된다. Wang 등<xref ref-type="bibr" rid="B20">20)</xref>은 개를 이용한 체외충격파 실험에서 골절의 신생골 형성과 피질골 형성이 대조군에 비해 더 형성되었고 체외충격파 치료의 효과는 시간 의존도가 있다고 보고하였으나, Forriol 등<xref ref-type="bibr" rid="B21">21)</xref>은 골절에 대한 체외충격파 치료가 골절 치유를 지연시킨다고 보고하였다. 이러한 정반대의 결과는 다른 동물에서 시행되었고 다른 용량의 충격파 때문인 것으로 생각된다.</p>
  <p>Wang 등<xref ref-type="bibr" rid="B22">22)</xref>은 토끼의 대퇴골 골절에 고용량의 체외충격파를 가했을 때 신생골 형성이 대조군에 비해 더 많았으며 뼈도 더 강하게 변했다고 보고하였고, 저용량의 체외충격파보다 효과가 좋다고 보고한 것 등을 볼 때 뼈에 대한 체외충격파 치료의 효과는 용량과 시간의 의존도가 높다고 생각된다.</p>

<sec>
<title>2. 건-뼈 접점 부위의 건병증(insertional tendinopathy)에서 조직 치유</title>
  <p>개의 아킬레스 건-뼈 접점 부위에 체외충격파를 가했을 경우 신생 혈관이 증가하는 것으로 보고되었고<xref ref-type="bibr" rid="B2">2)</xref> 토끼의 아킬레스 건-뼈 접점 부위에 체외충격파를 가했을 경우 혈관내피 산화질소 합성효소(endothelial nitric oxide synthase), 혈관내피 성장인자(vessel endothelial growth factor), 증식성 세포핵 항원(proliferative cell nuclear antigen) 등의 혈관 신생 인자 및 치유 인자가 많아짐을 확인하였다.<xref ref-type="bibr" rid="B5">5)</xref> 따라서 체외충격파가 가해지면 혈관 신생이 활발해지고 건-뼈 접점 부위에 혈액 공급이 많아져서 조직 치유에 도움을 주는 것으로 생각된다(<xref ref-type="fig" rid="F3">Fig. 3</xref>).<xref ref-type="bibr" rid="B1">1)</xref></p>
  <p>이외에도 과자극 진통(hyperstimulation analgesia)으로 통증 역치를 높여 통증이 조절된다는 이론도 제기되고 있다.<xref ref-type="bibr" rid="B1">1)</xref></p>

</sec>

<sec>
<title>체외충격파의 합병증과 금기 사항</title>
  <p>체외충격파 치료의 합병증으로는 통증, 어지럼증, 국소 출혈 등이 있으나 치료 중에 충격파의 강도를 조절하면 해결되며, 어지럼증을 호소하는 환자에서는 즉시 눕히고 다리를 높여주면 수분 내에 회복된다. 또한 큰 혈관이나 신경에 직접적으로 충격파가 가해지면 혈관 및 신경의 손상이 생길 수 있으므로 주의를 요한다.</p>
  <p>체외충격파의 금기증으로는 출혈성 질환이 있거나 항혈전 약물을 복용하는 경우 등이며 머리 부위, 소아의 성장판에 사용하는 것은 금기이다. 또한 폐에 충격파가 직접 전달되면 폐포(alveoli)가 손상될 수 있으므로 주의를 요한다.</p>
</sec>

<sec>
<title>석회성 건염</title>
  <p>석회성 건염에서의 체외충격파 치료는 통증과 석회 제거 효과에 유의한 치료 효과를 얻었다는 보고가 많으며 다양한 방식으로 체외충격파 치료를 시도하여 효과적인 치료 결과도 얻었다는 보고도 있다(<xref ref-type="table" rid="T1">Table 1</xref>). Wang 등<xref ref-type="bibr" rid="B6">6)</xref>은 견관절의 석회성 건염에 대한 체외충격파 치료로 57.6%에서 석회가 완전히 제거되었으며 15.1%에서는 석회가 부분적으로 제거되었고 27.3%에서는 석회에 변화가 없었다고 보고하였다. 또한 치료 후 2년 추시 결과 석회성 건염의 재발은 없었다고 하였다. Maier 등<xref ref-type="bibr" rid="B7">7)</xref>은 치료 후 자기공명영상 검사 결과 조직 손상 등의 체외충격파에 의한 부작용은 나타나지 않았다고 보고하였다. 석회성 건염에 대한 고용량의 체외충격파 치료가 저용량의 체외충격파 치료보다 우수한 효과가 있다는 메타분석 결과가 있으며,<xref ref-type="bibr" rid="B23">23)</xref> 체외충격파 치료 시 투시장치를 이용하여 석회침착이 되어 있는 부위에 초점을 맞추는 방법, 침착부위에 침을 놓는 방법, 어깨의 자세를 과신전, 내회전 상태에서 치료하는 방법 등이 보다 효과적인 방법이라는 보고도 있다. <xref ref-type="bibr" rid="B24">24</xref><xref ref-type="bibr" rid="B25">25</xref><xref ref-type="bibr" rid="B26">26)</xref></p>
</sec>

<sec>
<title>외상과염 및 내상과염</title>
  <p>외상과염 또는 내상과염에 대한 체외충격파 치료의 효과는 논란이 있다(<xref ref-type="table" rid="T2">Table 2</xref>). Rompe 등<xref ref-type="bibr" rid="B8">8)</xref>은 만성 외상과염에 3,000회의 충격파를 가했을 때, 30회의 충격파를 가한 대조군에 비해 좋은 임상적 결과를 보였다고 하였다. 이외에도 외상과염에 대한 체외충격파 치료가 효과가 있다는 보고가 많다.<xref ref-type="bibr" rid="B9">9</xref><xref ref-type="bibr" rid="B10">10)</xref> 그러나 체외충격파 치료의 결과가 유의하지 않다는 보고도 있으며<xref ref-type="bibr" rid="B27">27</xref><xref ref-type="bibr" rid="B28">28</xref><xref ref-type="bibr" rid="B29">29)</xref> 체외충격파 치료보다는 스테로이드 주사치료가 더 효과적이라는 보고도 있다.<xref ref-type="bibr" rid="B30">30)</xref> Dingemanse 등<xref ref-type="bibr" rid="B31">31)</xref>은 외상과염에 대한 여러 가지 전기물리치료 방법들의 효과에 대해서 분석하였는데 체외충격파 치료의 효과에 대해서는 논란이 있으며 초음파나 레이저 치료는 유의한 효과가 있다고 보고하였다.</p>
</sec>

<sec>
<title>슬개건염(Jumper's Knee) 및 아킬레스 건병증</title>
  <p>Wang 등<xref ref-type="bibr" rid="B11">11)</xref>은 슬개건염에 대한 체외충격파 치료 후 초음파 검사상 보존적 치료를 시행받은 군에 비해 충격파 치료군에서 혈관 재생이 더 많았고 슬개건이 얇아지며 치유되는 현상을 보였다고 보고하였으며, Peers 등<xref ref-type="bibr" rid="B12">12)</xref>은 만성 슬개건염에 대한 수술적 치료군과 체외충격파 치료군의 결과를 비교 분석하여 체외충격파 치료군에서 기능적 결과가 좋았다고 보고하였다. 아킬레스 건병증에 대한 체외충격파 치료도 좋은 결과를 보고하고 있다.<xref ref-type="bibr" rid="B13">13)</xref> Rompe 등<xref ref-type="bibr" rid="B32">32)</xref>은 만성 아킬레스 건병증 환자에서 스트레칭 치료군과 체외충격파 치료군을 비교하여 스트레칭 치료군의 결과가 더 나빴다고 보고하였다.</p>
</sec>

<sec>
<title>근위부 족저근막염</title>
  <p>족저근막염에 대한 체외충격파 치료로 좋은 결과를 얻었다는 보고는 많으며 치료 성공률은 88%까지 보고되고 있다.<xref ref-type="bibr" rid="B14">14)</xref> Rompe 등<xref ref-type="bibr" rid="B15">15)</xref>은 족저근막염 환자에 0.16 mJ/mm<sup>2</sup> 용량으로 1주마다 6,300회의 충격파를 3번에 걸쳐 치료한 결과 좋은 결과를 얻었다고 보고하였다. 또한 Wang 등<xref ref-type="bibr" rid="B16">16)</xref>은 족저근막염에 체외충격파를 시행한 결과 효과가 없거나 나빠진 환자는 없었고, 재발률은 5%였다고 보고하였다. 그러나 족저근막염에 체외충격파 치료의 효과가 없었다는 보고도 있다.<xref ref-type="bibr" rid="B33">33)</xref> Aqil 등<xref ref-type="bibr" rid="B17">17)</xref>이 시행한 메타분석에 따르면 체외충격파 치료는 대조군에 비해 유의한 치료효과를 보여 3개월 이상 비수술적 치료에 효과가 없는 만성 족저근막염 환자군에서 체외충격파 치료를 사용할 것을 권하였다.</p>
</sec>

<sec>
<title>근막통증 증후군</title>
  <p>근막통증 증후군은 골격근에 통증 유발점이 생겨 심한 통증 및 기능 장애를 나타내는 질환으로서 여러 가지 치료가 시행되고 있으나 체외충격파 치료를 통증 유발점에 시행함으로써 좋은 임상적 결과를 보였다는 보고가 최근에 늘어나고 있다.<xref ref-type="bibr" rid="B34">34)</xref> 저자도 후견갑부의 만성 근막통증 증후군에서 체외충격파 치료로 좋은 결과를 얻고 있다.<xref ref-type="bibr" rid="B35">35)</xref></p>
</sec>

<sec sec-type="conclusions">
<title>결론</title>
  <p>근골격계 질환에 대한 체외충격파 치료의 많은 보고들을 볼 때, 이 치료는 비침습적이고 적응증을 잘 선택하면 치료 효과가 좋으며 합병증의 예방이 가능하고 생겨도 심하지 않아서 안전한 치료로 볼 수 있고 수술적 치료 방법을 선택하기 전에 시행될 수 있기 때문에 유용하다고 할 수 있다. 체외충격파 치료의 원리인 기계적인 에너지 집중 효과와 생물학적 효과의 적응증을 잘 선택해서 시행한다면 좋은 임상 결과를 얻을 수 있을 것으로 생각된다.</p>
